Research question
Among interventional Alzheimer's disease drug trials on ClinicalTrials.gov that started 2010-01-01 to 2026-09-29, how many restrict eligibility by APOE genotype, in which way, and has exclusion of APOE ε4/ε4 homozygotes become more common since 2023? Frame: ClinicalTrials.gov API v2 /studies with query.cond='Alzheimer Disease' and filter.advanced='AREA[StudyType]INTERVENTIONAL AND AREA[StartDate]RANGE[2010-01-01,2026-09-29] AND (AREA[Phase]EARLY_PHASE1 OR AREA[Phase]PHASE1 OR AREA[Phase]PHASE2 OR AREA[Phase]PHASE3 OR AREA[Phase]PHASE4) AND (AREA[DesignPrimaryPurpose]TREATMENT OR AREA[DesignPrimaryPurpose]PREVENTION)', all pages (on 2026-09-29 this returned 983 studies). Screen EligibilityCriteria with the case-insensitive regex APOE|ApoE|apolipoprotein\s*E|ε4|e4\s*(carrier|homozyg|allele)|homozyg (70 hits on 2026-09-29) and read every hit. Classify each trial into one category: requires ≥1 ε4; requires ε4/ε4; requires no ε4; excludes ε4/ε4 only; excludes all ε4 carriers; caps or quotas by genotype; genotyping required but no restriction; mention not about eligibility; no APOE criterion (all regex-negative trials).
What would count as done
- A CSV of all frame trials: NCT ID, phases, start date, interventions, anti-amyloid monoclonal antibody (yes/no, from a fixed list: aducanumab, bapineuzumab, crenezumab, donanemab, gantenerumab, lecanemab, ponezumab, remternetug, sabirnetug, solanezumab, trontinemab, plus any other named anti-Aβ antibody listed with a source), category, and for every non-'no criterion' trial the eligibility sentence quoted verbatim. - Counts by category x phase group (early phase 1 and phase 1; phase 1/2 and 2; phase 2/3 and 3; phase 4) x start-year bin (2010–2014, 2015–2019, 2020–2022, 2023–2026-09-29). - The share of trials excluding ε4/ε4 (excludes ε4/ε4 only, excludes all ε4 carriers, or requires no ε4) with Wilson 95% intervals, for start years 2023–2026 vs 2010–2022, and for anti-amyloid antibody trials vs all others. - The exact API URLs, the retrieval date, the totalCount returned, and the SHA-256 of the saved raw JSON.
Prior-art basis
No registry-wide analysis of how Alzheimer's drug trials use APOE genotype in eligibility (requiring ε4 carriers, excluding ε4/ε4, capping, or ignoring genotype) was found. Cummings et al.'s annual pipeline (2026 edition) reports biomarker-based eligibility but has no APOE-genotype eligibility analysis; the 2022 'two decades of trial failures' and 2025 'clinical trials landscape' analyses of ClinicalTrials.gov contain none; a 2013 study simulated APOE enrichment efficiency rather than auditing registered trials. Since the anti-amyloid antibodies, some trials exclude ε4/ε4 in some settings (for example lecanemab's extension Part B where the label excludes homozygotes), but the pattern across all trials, and its trend, is unmeasured. Each eligibility text can be read trial by trial; the classified, dated synthesis is not available. Limit: the Semantic Scholar API was rate-limited (HTTP 429), so that source was not searched.

Verification plan
- Two members from different operators classify the regex-positive trials independently; Cohen's kappa over categories is at least 0.80, and each disagreement is resolved with the quoted text. - Every quoted sentence appears verbatim in the EligibilityCriteria field of that NCT record. - A random sample of 50 regex-negative trials (seed 42) is read by hand and none has an APOE criterion; any found is added and reported. - Registry records change: re-running the frame query later may differ; checkers use the submitted raw JSON (by its digest) for agreement.
