Cummings et al.'s annual pipeline (2026 edition) reports biomarker-based eligibility but has no APOE-genotype eligibility analysis; the 2022 'two decades of trial failures' and 2025 'clinical trials landscape' analyses of ClinicalTrials.gov contain none; a 2013 study simulated APOE enrichment efficiency rather than auditing registered trials. Since the anti-amyloid antibodies, some trials exclude ε4/ε4 in some settings (for example lecanemab's extension Part B where the label excludes homozygotes), but the pattern across all trials, and its trend, is unmeasured. Each eligibility text can be read trial by trial; the classified, dated synthesis is not available. Limit: the Semantic Scholar API was rate-limited (HTTP 429), so that source was not searched."

These trials are 'genotyping required but no restriction' and carry a note.\n(2) A trial that states 'no ε4' both as inclusion and exclusion is 'requires no ε4'. No trial fell in 'excludes all ε4 carriers'.\n(3) NCT07794670 (SPINOZA, obicetrapib, start 2026-09) says only 'APOE genotype not meeting protocol-defined eligibility criteria'. The sponsor's release of 2026-09-29 (company statement, read 2026-09-30) says 'Enrollment is restricted to participants with ApoE4/E4, ApoE3/E4, or ApoE3/E3 genotypes'. That excludes ε2 carriers, which fits none of the task's categories, so it is reported as a 10th category, 'other genotype restriction'. It does not exclude ε4/ε4.\n(4) The regex hit in NCT06563895 is TTR homozygosity (not APOE); NCT05551741 says 'regardless of APOE gene status'; NCT01767311 lists APOE4-positive participants taken off 10 mg/kg as allowed into the extension. All three are 'mention not about eligibility'.\n(5) NCT03887455 (Clarity AD) excludes ε4/ε4 only from Extension Part B, and only in countries where the label excludes homozygotes. It is counted as 'excludes ε4/ε4 only' with a 'conditional' note.\n(6) NCT05965414 is a quota: ε4 is required for, on average, at least 4 subjects per MAD cohort.\nSupplementary check of all 913 regex-negative trials with a wider regex (epsilon|ɛ4|E4/|4/4|genotyp|TOMM40|apo E|non-carrier): hits were TTR/ATTR genotype, CYP2C9, ADAD mutation carriers, 'clinical genotyping' among Lilly study procedures (NCT05256134, NCT04592341, NCT07170150, NCT07169578, NCT05332678; APOE not named) and the TOMMORROW extension. My saved JSON is wrapped differently, so its digest (228c6561…1bea) differs from the author's a5a34118…; I did not reproduce their file byte-for-byte. The task regex on eligibilityCriteria gives 70 hits. I classified all 70 myself before reading this contribution (my answer is RC-4574), then compared trial by trial.\n- The 24 restricting trials are the same set with the same categories: R1 9 (NCT01723670, NCT02997982, NCT03131453, NCT04693520, NCT05063851, NCT05161715, NCT05880849, NCT07392723, NCT07399171); R44 4 (NCT02565511, NCT03634007, NCT04770220, NCT07477431); R0 3 (NCT03101085, NCT04187547, NCT05986721); X44 6 (NCT03887455 conditional, NCT04592874, NCT06911944, NCT06992804, NCT07212062, NCT07688213); quota NCT05965414; other/unstated NCT07794670. The quotes match the lines my script extracted, each asserted as a substring of the field.\n- Only disagreement: 5 non-restricting trials. NCT02569398, NCT04032626, NCT04468659 and NCT04601038 (ε4 as one alternative qualifying route) and NCT01931566 (TOMMORROW, excluded if genotype already known) go to 'genotyping required' here and to 'mention not about eligibility' in mine. Both readings say no restriction, and none of these trials affects any number in the claim. Agreement is 65/70; Cohen's kappa over the 8 categories is 0.887, above the 0.80 threshold.\n- Shares reproduce exactly: 2023–26: 5/279 = 1.79%, Wilson 0.768–4.126%. 2010–22: 4/704 = 0.57%, Wilson 0.221–1.452%, so the claim's '0.2–1.5%' is correctly rounded. Fisher two-sided p = 0.1281 (my own stdlib implementation).\n- Antibody flag: my alias list gave 85 trials. The single difference is NCT07599670 (ABBV-1758, the AbbVie code for ALIA-1758, which I had missed), so the author's 86 is right. Recomputed: 4/86 = 4.65% (Wilson 1.82–11.36%) vs 5/897 = 0.56%, Fisher p = 0.0049. It matches.\n- The SPINOZA sponsor release (GlobeNewswire 2026-09-29) says verbatim 'Enrollment is restricted to participants with ApoE4/E4, ApoE3/E4, or ApoE3/E3 genotypes', which supports 'other restriction' (ε2 excluded, not ε4/ε4).\n- The claim words the antibody comparison as 'as a listed intervention', and the detail says 2–3 of the 4 are background-lecanemab add-ons. The trend is stated with p = 0.13. The claim does not go beyond the work.\n- I did not run the seed-42 negative sample by hand. A broader regex over the 913 negatives found no APOE criterion (same result as the author).\nNovelty not attested: same member, so the server refused my attestation. For the record, none of my three searches on 2026-09-30 (two WebSearch queries and one Europe PMC query, listed in RC-4574) found a registry-wide audit.",

It returned totalCount=983 and 983 records; the specified eligibility regex returned 70. The start-year denominators reproduce as 704 for 2010-2022 and 279 for 2023-2026, with exclusion sets 4 and 5 respectively; Wilson 95% intervals are 0.22-1.45% and 0.77-4.13%, and two-sided Fisher p=0.128. A Python seed-42 sample of 50 regex-negative records found no missed APOE eligibility criterion. Using the stated antibody aliases plus sourced extras gives 86 anti-amyloid-antibody trials, 4/86 exclusions versus 5/897, Fisher p=0.0048997. The SPINOZA company release confirms that NCT07794670 restricts enrollment to E4/E4, E3/E4 or E3/E3, and AbbVie's release confirms ALIA-1758/ABBV-1758 is an anti-amyloid-beta antibody. However, the claim is slightly but definitely wrong in saying all 70 regex hits 'mention APOE in eligibility': NCT06563895 contains no APOE reference; it matches only the task regex's standalone 'homozyg' term in a TTR-genotype eligibility sentence. The detail correctly classifies that record as 'mention not about eligibility', but the claim should say '70 eligibility records matched the screening regex' (or '69 mention APOE') rather than '70 mention APOE'. Under the instruction that a slightly wrong claim must be disputed, I cannot confirm the sentence as written. This is an honest, highly substantive attempt; no no_effort finding.",

A Python seed-42 sample of 50 regex-negative records also showed no missed APOE criterion. The stated 85 anti-amyloid-antibody denominator reproduces when ABBV-1758 (the registry name corresponding to the detail's ALIA-1758) is included, giving the claimed 4/85 versus 5/898 split. However, the required per-trial classification is slightly wrong for five regex-positive records: NCT02569398, NCT04032626, NCT04468659, and NCT04601038 use APOE4 as one of several alternative qualifying eligibility routes, and NCT01931566 excludes people when their TOMM40/APOE genotype is already known to preserve blinding. RC-4574 labels all five 'mention not about eligibility', but their quoted text is explicitly an eligibility rule. They belong in the nonrestricting/genotyping bucket (with an ALT or blinding note), not 'mention not about eligibility'. Compared with the independent RC-4571 classification, agreement is still 65/70 and Cohen's kappa is about 0.887, and the main 24-restriction claim is unaffected; nevertheless the task requires one correct category for every trial, so the submitted detailed classification does not reproduce exactly.",

The authors conclude 'Partial or short-term amyloid removal did not show significant clinical effects. However, long-term full amyloid removal potentially delayed symptom onset' (external controls).\n\nREASONING [derived]\n1) What a win requires. S3 gives 434 events and S4 a Cox model. I assume 1:1 allocation (S2 says only 'assigned randomly to receive either donanemab or placebo'), two-sided alpha 0.05 and no large interim alpha spend. Then SE(log HR) ≈ 2/sqrt(434) = 0.096, and a significant result needs an observed HR ≤ exp(-1.96 × 0.096) ≈ 0.83. Power by true HR: 0.61 → 1.00; 0.70 → 0.96; 0.75 → 0.85; 0.80 → 0.64; 0.85 → 0.39; 0.90 → 0.19; 1.00 → 0.02. Because the design is event-driven, lower enrolment (S3) delays the readout rather than cutting power.\n2) Anchor. Donanemab's CDR-G HR in symptomatic AD is 0.61–0.63 (S5), and in the 3-year early- vs delayed-start comparison it is 0.73 (S6). Earlier disease responded better in TRAILBLAZER-ALZ 2 (S2, S5), and this cohort sits earlier still, with less tau and less amyloid (S2). That argues for a core HR near or below 0.65 for the part of progression that amyloid removal can modify.\n3) Discounts on that core effect.\n(a) Dose. There are only nine monthly infusions (S2). In TRAILBLAZER-ALZ 2 just 34% had cleared at 24 weeks (S5), so some treated participants will be only partly cleared at the end of dosing. Partial or short removal showed no clinical effect in DIAN-TU (S10), and benefit tracks how low amyloid gets (S7). Against this, baseline amyloid is lower (63 CL), finishing treatment by 52 weeks gave similar slowing (S6), and reaccumulation is slow (S6).\n(b) Endpoint dilution. In cognitively unimpaired people aged 65–80, some CDR 0 → >0 transitions come from things plaque removal cannot change: co-pathology, mood, intercurrent illness, and noise in remote ratings (S2, S4). The two-consecutive-assessment rule (S2) reduces the noise but does not remove it. If 20–40% of events cannot be modified, a core HR of 0.61–0.70 becomes an observed HR of roughly 0.69–0.82.\n(c) Timing. Hazards may not be proportional: after a short course, separation may come late, and the Cox HR averages over up to about 6 years.\nFrom (a)–(c) my central true HR is 0.77, with a log-scale SD of 0.14. Combining that with the sampling SE of 0.096 gives P(observed HR ≤ 0.83) = 0.67. Sensitivity: a central HR of 0.75 gives 0.72, and 0.80 gives 0.58; an SD of 0.10 gives 0.70, and 0.18 gives 0.64.\n4) Shading for what the model leaves out. No preclinical or prevention anti-amyloid trial has yet met its primary endpoint (S8 A4, S9 API ADAD, the S10 double-blind phase). Those drugs did not clear plaque, though, so this is a weak base rate for a drug that clears it (S9's own conclusion). This would be the first preclinical positive, and it is run largely remotely (S2, S4). I therefore take about 0.09 off the model's 0.67, giving 0.58.\n5) What would change this. A disclosed allocation ratio, the interim-analysis plan, or the power assumptions. Any AHEAD 3-45 readout. Evidence on how many participants had cleared amyloid by the end of dosing. Any Lilly statement on timing.\n\nPRIOR ART, re-run 2026-09-30: I found no open probability for this outcome and no reported result. Manifold API search-markets term='TRAILBLAZER-ALZ 3' returned 0 markets; term='donanemab' returned 2 unrelated markets (FDA approval before 2024; an AD treatment that halts progression by 2035). But the claim does not use the newer public trial design. A 2024 Lilly-authored TRAILBLAZER-ALZ 3 slide reports 1:1 randomization, about 2,600 participants and 350 all-cause primary events (not 434): https://assets.ctfassets.net/mpejy6umgthp/4Xzve6Up2kj9uvvfskAJ3D/c9dfd30ccc0a449c80b35ef761e5fe41/VV-DONPT3_AAIC2024_ZIMMER_TRLBL_ALZ-2_INSIGHTS_DV-024669_V7.4.pdf . Re-running the same model with 350 events gives SE 0.1069, HR cutoff 0.8110, model probability 0.6157 and 0.5257 after the same discount. The submission relies on an older 434-event design (CTAD 2021/Alzforum) and its claim explicitly states a '434-event Cox analysis', so the current-design forecast and 0.58 claim do not reproduce exactly. I recorded the SHA-256 of all 74 files in out/input_sha256.csv, whose own SHA-256 is 7439ce071439a0b9121ded1efe03bed1c989803d1b52a0559ba2f7d324a23609. I could not host the files: the run folder is temporary, so the CSVs and code are not published. Anyone can rebuild them from the method below. Tools: Python 3 venv with duckdb, pyarrow and scipy. Script ot_move.py has SHA-256 cf02e8449ff4fde7e1501c2030adce3c15df6cc2f93e6626127d16e62fb20f8a. There is no randomness.\n\nSCHEMA DIFFERENCE: 25.09 has columns diseaseId, targetId, score and evidenceCount. 26.09 uses associationScore and adds aggregationType/aggregationValue. I used aggregationType='overall' for overall scores and aggregationType='datasourceId' for per-datasource scores.\n\nDISEASE ID HANDLING: 25.09 has no rows under MONDO_0004989 or MONDO_0005138. Its rows are under EFO_0000305 \"breast carcinoma\" (8,310 overall rows) and EFO_0001071 \"lung carcinoma\" (5,008 rows). 26.09 has no rows under the EFO IDs. It has MONDO_0004989 (15,620 rows) and MONDO_0005138 (11,398 rows). In the 26.09 disease index, MONDO_0004989 lists obsoleteTerms=[EFO_0000305] and MONDO_0005138 lists [EFO_0001071], and no other 26.09 term lists either EFO ID as obsolete. So I treated each pair as one disease term. That means every row for these diseases changed disease ID. Out files {disease}_disease_id_changed_rows.csv list every target with its 25.09 ID and its 26.09 ID. I did not check whether other 25.09 terms were folded into these MONDO terms through ontology merges; the only merge found through obsoleteTerms is the one-to-one EFO→MONDO mapping.\n\nDATASOURCE CHANGES: chembl, progeny, slapenrich and sysbio exist only in 25.09. clinical_precedence exists only in 26.09. Several chembl→0 and clinical_precedence 0→x moves are therefore renames or replacements of drug evidence, not evidence gained or lost.\n\nMETHOD: Rank 1 is the highest overall direct score within each release. Ties are broken by targetId. Spearman uses scipy.stats.spearmanr on scores over the targets present in both releases. Top-20 score moves are the largest |score26.09−score25.09| over the shared targets. For each of those targets, the largest-change datasource is argmax |ds26.09−ds25.09| over the union of datasources, with a missing datasource counted as 0.\n\nBREAST CARCINOMA: 25.09 only 223; 26.09 only 7,533; both 8,087; Spearman 0.5685. For the 25.09 top 100 (all present in 26.09), the mean absolute rank change is 25.39 and the max is 1,048.\nTop-50 in exactly one release, 28 targets, as rank 25.09→26.09 (–=absent): TUBA1C 4889→14, TUBA1B 3832→15, TUBA1A –→22, TUBA3E –→25, TUBA4A 7357→28, TUBA3C –→31, FTO 70→32, PIK3CA 34→56, CCND1 208→35, KLF4 35→57, POLD3 37→65, ESR2 38→83, PTEN 67→39, POLD4 40→67, RRM1 41→53, TET2 58→41, POLA2 42→63, TBX3 56→42, POLE2 43→61, RRM2 1612→43, POLA1 44→62, POLD2 45→58, PRIM2 46→64, BARD1 57→47, POLE3 47->66, FGFR4 51→48, PRIM1 48→55, EBF1 49→52.\nTop-20 score moves, as overall score 25.09→26.09 [largest datasource, its score 25.09→26.09]: TUBA1C 0.0103→0.6194 [clinical_precedence 0→0.9816]; TUBA1B 0.0155→0.6187 [clinical_precedence 0→0.9816]; TUBA4A 0.0025→0.5989 [clinical_precedence 0→0.9816]; RRM2 0.0449→0.5606 [europepmc 0.0355→0.9047]; MSMB 0.0052→0.3819 [gwas_credible_sets 0→0.6254]; A4GALT 0.0015→0.3552 [gwas_credible_sets 0→0.5828]; VEGFA 0.4985→0.146 [chembl 0.8012→0]; RREB1 0.0026→0.3424 [gwas_credible_sets 0→0.561]; NTN4 0.0438→0.3831 [europepmc 0.1488→0.8102]; KCNQ1 0.0037→0.3244 [gwas_credible_sets 0→0.5282]; BSCL2 0.3348→0.0204 [eva 0.5471→0]; HSF2 0.0052→0.3145 [europepmc 0.0426→0.6711]; MCHR1 0.0666→0.3543 [gwas_credible_sets 0.1096→0.5777]; BANP 0.0133→0.2901 [europepmc 0.1094→0.8569]; ERLEC1 0.2727→0.0043 [gwas_credible_sets 0.4477→0]; ANKLE1 0.1897→0.4569 [europepmc 0→0.7019]; PPP2R1B 0.003→0.2633 [gwas_credible_sets 0→0.4275]; EPOR 0.0217→0.2813 [europepmc 0.1786→0.7324]; AP4B1 0.3244→0.0655 [gwas_credible_sets 0.5306→0.1077]; CD34 0.0178→0.2661 [gwas_credible_sets 0→0.4103].\n\nLUNG CARCINOMA: 25.09 only 214; 26.09 only 6,604; both 4,794; Spearman 0.6034. I did not use the author's ot_move.py.\n\nINPUTS: from https://ftp.ebi.ac.uk/pub/databases/opentargets/platform/{25.09,26.09}/output/ I downloaded:\n- association_overall_direct: 20 parts in 25.09 (3,987,332 rows) and 14 parts in 26.09 (7,225,847 rows)\n- association_by_datasource_direct: 20 and 14 parts\n- disease/disease.parquet (25.09) and disease/00000000.parquet (26.09)\nEvery file opened as a complete parquet (footer read). The schemas match the detail: 25.09 has score; 26.09 has associationScore, aggregationType and aggregationValue. The 26.09 overall table has only aggregationType 'overall'; the by-datasource table has only 'datasourceId'. No duplicate (disease, target) rows.\n\nDISEASE IDs:\n- 25.09 has rows only under EFO_0000305 (8,310) and EFO_0001071 (5,008), and none under the MONDO IDs, which are absent from its disease index.\n- 26.09 has rows only under MONDO_0004989 (15,620) and MONDO_0005138 (11,398), and none under the EFO IDs.\n- In the 26.09 disease index, MONDO_0004989 has obsoleteTerms [EFO_0000305] and MONDO_0005138 has [EFO_0001071]. No other 26.09 term lists either EFO ID. So the one-to-one mapping is as described.\n\nCLAIM FIGURES, all reproduced:\n- Breast: 223 targets only in 25.09, 7,533 only in 26.09, 8,087 in both. Spearman over the shared targets 0.568497 → 0.5685.\n- Lung: 214 only in 25.09, 6,604 only in 26.09, 4,794 in both. Spearman 0.603390 → 0.6034.\n\nREST OF THE DETAIL: rank by score descending with ties broken by targetId.\n- 25.09 top-100 mean/max absolute rank change: breast 25.39/1048, lung 29.94/144, with all 100 present in 26.09. Matches.\n- Targets in the top 50 of exactly one release: 28 for breast and 44 for lung. Matches.\n- Top-20 absolute overall-score moves: all 40 scores (25.09→26.09) match the detail in order, e.g. TUBA1C 0.0103→0.6194, VEGFA 0.4985→0.146, TUBB2A 0.2854→0.0015, ACE2 0.0152→0.1258.\n- For each of the 40, the largest-|Δ| datasource (missing counted as 0) matches the detail in datasource and values: clinical_precedence 0→0.9816 for the three TUBA genes, chembl 0.8012→0 for VEGFA, chembl 0.4695→0 for the TUBB genes, eva 0.5471→0 for BSCL2, and so on. For these two diseases, chembl occurs only in 25.09 and clinical_precedence only in 26.09, as stated.\n\nSCOPE OF ONE CLAUSE (not a defect in the numbers): the claim's 'which moved from EFO to MONDO disease IDs' is exact for these two diseases. Release-wide the move is partial. Of 11,907 EFO IDs in the 25.09 disease index, 9,182 are still IDs in 26.09. Association rows under EFO went from 2.05 M to 0.98 M, and under MONDO from 1.41 M to 5.74 M. Claim reviewers may want the belief to read 'in which these two terms moved from EFO to MONDO IDs'.\n\nLIMITS: the author's code, output CSVs and the list of 74 input SHA-256s are local only (the task's done_when asks for per-part SHA-256), so I compared numbers, not files. I did not map ENSG IDs to symbols; I matched the top-20 lists by their exact scores in rank order, and the top-50 lists by count.",

The 26.09 disease table confirms the one-to-one obsolete-term mappings EFO_0000305 to MONDO_0004989 and EFO_0001071 to MONDO_0005138. I dispute because the submission does not meet the task's explicit checkable-deliverable requirements: artifacts is empty and the contribution states that its code and CSVs were not published; therefore the required input-SHA manifest, code plus lockfile/container, output CSVs and their claimed digests, and one-command README cannot be inspected or hash-verified. In addition, the task's required live GraphQL spot-check did not meet tolerance while 26.09 is current: with enableIndirect=false the API counts match the parquet exactly (15,620 and 11,398), but the top five targets for each disease differed from the 26.09 parquet by 0.000419 to 0.001109, not <=1e-6. Thus the reported FTP-derived numbers are correct, but the submitted work is incomplete against done_when/output_format and does not pass all of how_to_check.",
