"claim_statement": "On evidence published by 31 Dec 1987, eradicating C. pyloridis was the single most likely approach to be the recommended standard cure for duodenal ulcer by the end of 1997 (p≈0.40). It was not more likely than not, because maintenance acid suppression (p≈0.35) was the established default and the eradication-relapse link rested on one observational cohort of 39 patients.",

"detail": "METHOD: I fetched PubMed abstracts (NCBI efetch) for every PMID below and read the figures quoted here in the abstracts. All sources are dated 1981 to Dec 1987. I searched PubMed with a publication-date limit of 1983-1987 for early omeprazole trials, with the queries \"omeprazole AND duodenal ulcer AND (relapse OR recurrence)\" and \"omeprazole AND duodenal ulcer AND ranitidine\". The outcomes are treated as mutually exclusive answers to \"which is THE recommended standard for curing DU\", and there is a residual bucket. HINDSIGHT: I know what happened after 1987. I did not use it for any probability, and no post-1987 source is cited.\n\n(a) ERADICATION OF C. PYLORIDIS: p = 0.40\nFor:\n- Relapse after healing tracked the organism. Coghlan, Lancet 14 Nov 1987 (PMID 2890019), n=39: 12-month relapse was 10% if the patient stayed C. pylori-negative, 66% if the organism recurred, 79% if culture-positive and 27% if culture-negative.\n- A short course of bismuth, which suppresses the organism, relapsed less than H2 antagonists in 3 RCTs:\n - Miller/Martin 1981-82 (PMID 6761851, 6109102): 39% vs 85% at 12 months.\n - Hamilton, Gut 1986 (PMID 3512382): 43% vs 78%.\n - Lee, Lancet 1985 (PMID 2860494): 62% vs 89%.\n- Bismuth plus amoxycillin or tinidazole gave long-term eradication and healing of gastritis, whereas the infection relapsed after bismuth alone. Marshall, Digestion 1987 (PMID 3622946).\n- The effect implied by Coghlan (about 10% vs about 70%) is large. A randomised relapse trial would need only about 20-30 patients per arm, and 1987 tools could deliver it within 2-3 years. That leaves most of the decade for replication and adoption, so if the hypothesis is true, adoption by 1997 is plausible.\nAgainst:\n- The link comes from one observational cohort, and there is no randomised eradication-relapse trial.\n- Eradication was hard with the regimens then available. Graham & Klein 1987 (PMID 3551584).\n- Infection is common in people without ulcers: seropositivity in healthy subjects, Kaldor 1986 (PMID 3526100).\n- Ulcers healed on cimetidine while the organism persisted. Hui 1987 (PMID 3665680).\n- Koch's postulates had been met only for gastritis.\n- Adoption would be slowed by the acid paradigm and by commercial support for H2 antagonists.\nEstimate: P(the causal role holds and eradication is decisively better) ≈ 0.55. P(it becomes the recommended standard within 10 years, given that) ≈ 0.75. 0.55 × 0.75 ≈ 0.41, rounded to 0.40.\n\n(b) LONG-TERM OR MAINTENANCE ACID SUPPRESSION: p = 0.35\nFor:\n- It was already established in 1987. Freston, Am J Gastroenterol Dec 1987 (PMID 2891294): maintenance H2 antagonists cut annual recurrence from nearly 70% to about 25%. Its duration was uncertain.\n- Thomson & Mahachai 1987 (PMID 2887321) describe cimetidine and ranitidine as the standard, with famotidine and omeprazole coming.\n- Omeprazole healed faster. Lauritsen, NEJM 1985 (PMID 3883182): 92% vs 74% healed at 4 weeks.\n- It is the default if eradication fails to replicate.\nAgainst:\n- It does not cure. Relapse after stopping omeprazole was 56% symptomatic at 12 months, similar to H2 antagonists. Brook, Med J Aust Dec 1987 (PMID 3320709). Lauritsen 1985 found no difference in recurrence from cimetidine at 6 months.\n- Omeprazole trials were suspended in 1984 after gastric carcinoids in rats (PMID 3883182, 2887321), which clouds lifelong use.\n- Lifelong treatment is costly, and Freston shows its benefit stops when it is stopped.\nThis bucket scores high because it is the incumbent, not because it cures.\n\n(c) ELECTIVE SURGERY: p = 0.05\nRecurrence after highly selective vagotomy was:\n- 11.6% at 5 years and 16.8% at 10 years, varying by surgeon. Adami, Ann Surg 1984 (PMID 6712313).\n- 30% proven plus 9% suspected at 14-18 years after parietal cell vagotomy. Hoffmann, Br J Surg 1987 (PMID 3690236).\nMore radical operations had fewer recurrences but carried morbidity and mortality. De Vries 1983 (PMID 6357339); Donahue 1984 (PMID 6385314). Effective drugs were already reducing elective surgery. A drug failure is unlikely to reverse that trend.\n\n(d) NO SINGLE STANDARD OR OTHER: p = 0.20\nThis bucket covers intermittent short courses with retreatment, bismuth monotherapy, prostaglandin analogues (Thomson 1987, PMID 2887321), or no consensus on a \"cure\" by 1997.\n\nSum: 0.40 + 0.35 + 0.05 + 0.20 = 1.00.\nCompared with RC-4555 (codex): I have not relied on it. My structure is similar, with eradication leading but short of a majority.",

"note": "I retrieved all 17 cited PubMed abstracts via NCBI efetch: 15 in one batch, plus 6712313 and 3690236 earlier for the CL-65 review. All exist and are dated between Jan 1981 (6109102) and Dec 1987 (2891294, 3320709). No source is after the cutoff. Every quoted figure matches its abstract:\n- Coghlan (2890019): n=39. Relapse was 79% if culture-positive vs 27% if culture-negative, 66% if C. pylori recurred vs 10% if it stayed negative.\n- Miller 1982 (6761851): 39% vs 85% at 1 yr. Martin 1981 (6109102) is the same trial and its abstract gives no numbers.\n- Hamilton (3512382): 43% vs 78%.\n- Lee (2860494): 62% vs 89% at 12 months.\n- Marshall (3622946): bismuth plus amoxycillin or tinidazole gave long-term clearance, and relapse was common after bismuth alone.\n- Graham & Klein (3551584): 'difficult to eradicate'.\n- Kaldor (3526100): the same immune response is seen in apparently healthy people.\n- Hui (3665680): ulcers healed on cimetidine while the organism persisted.\n- Freston (2891294): about 70% falls to about 25% a year, duration uncertain.\n- Thomson & Mahachai (2887321): cimetidine is the standard, with the carcinoid concern about omeprazole and prostaglandin analogues.\n- Lauritsen (3883182): 92% vs 74% at 4 weeks, no difference in recurrence at 6 months, trials suspended in May 1984.\n- Brook (3320709): 56% symptomatic relapse at 12 months, similar to H2 antagonists.\n- Adami (6712313): 11.6% and 16.8%. Hoffmann (3690236): 30% proven plus 9% suspected.\n- De Vries (6357339) and Donahue (6385314): more radical operations had fewer recurrences but more morbidity (1 TVA death; 26% serious digestive problems after SVA).\n\nThe arithmetic holds: 0.55 x 0.75 = 0.41, rounded to 0.40, and 0.40+0.35+0.05+0.20 = 1.00. The probabilities follow from the cited 1987 evidence, and no hindsight is used as evidence. The claim that eradication was most likely but not a majority, with the eradication-relapse link resting on one observational cohort of 39, is supported: Coghlan is the only cited pre-1988 cohort linking eradication status to relapse. Two small caveats, neither enough for partial: the surgical recurrence figures come from mixed prepyloric and duodenal cohorts (see CL-65), and the 'Miller/Martin' pair is one trial reported twice, which the contribution counts correctly as one of 3 RCTs.",
