"claim_statement": "In the literature published by 1987, duodenal ulcers relapsed within 12 months of stopping treatment in 78-89% of patients after H2 antagonists (DU-only trials) and 39-62% after bismuth. Relapse was 16-43% on maintenance H2 antagonists. It was 10% in patients who stayed C. pylori-negative against 66-79% in those with persistent or recurrent infection (one observational cohort, n=39), and no randomised eradication-relapse trial was found. Recurrence after proximal gastric vagotomy was 0-20% at 4-13 years, rising to 30% at 14-18 years.",

"detail": "METHOD: I retrieved all 20 distinct PMIDs cited by the two results through NCBI efetch (abstracts) and checked each figure and date against its abstract. Every source is dated from 3 Jan 1981 to Nov 1987, so none falls after the cutoff.\n\nWHERE THEY AGREE (both reproduce exactly):\n- (a) Stopping an H2 antagonist after healing: Miller 1982 (PMID 6761851) gives cimetidine 85% at 12 mo, and it is the same trial as Martin 1981 (PMID 6109102). Lee 1985 (PMID 2860494) gives ranitidine 89%.\n- (c) Bismuth: Miller gives CBS 39%, Lee gives TDB 62%.\n- (e) C. pylori: Coghlan 1987 (PMID 2890019), n=39, reports 59% overall, 79% if culture-positive, 27% if culture-negative, 66% if the organism recurred after apparent eradication and 10% if it stayed negative. Both results correctly call this an observational stratification, not a randomised trial. Both report finding no pre-1988 randomised eradication-relapse trial. Marshall 1987 (PMID 3622946) shows CBS plus amoxycillin or tinidazole giving long-term clearance of the organism, with no relapse percentage.\n- Hoffmann 1987 (PMID 3690236): 30% proven plus 9% suspected recurrence at 14-18 yr after parietal cell vagotomy.\n- Both conclude that acid suppression is not a cure and that clearing C. pylori is the best non-surgical signal of a lasting cure, on small observational data.\n\nWHERE THEY DIFFER:\n1. The sources differ in coverage. RC-4554 adds Hamilton 1986 (PMID 3512382: TDB 43% vs cimetidine 78%), Silvis 1985 (PMID 3910710: maintenance ranitidine 16% vs cimetidine 43%), Liedberg 1985 (PMID 3909374: 16% vs placebo 72%, mixed ulcer sites), Lam 1987 (PMID 3557014: 50% relapse at 12 mo after sucralfate vs 6 mo after cimetidine) and Bolin 1987 (PMID 3661614: maintenance sucralfate 10/24 vs placebo 21/26). RC-4557 adds Alstead 1983 (PMID 6301956: the only DU-specific placebo-controlled maintenance figure, 7/20 = 35% vs 15/17 = 88%) and the randomised surgical trials Donahue 1984 (PMID 6385314: selective vagotomy plus antrectomy 0%, PGV 20% at 4-12 yr) and De Vries 1983 (PMID 6357339: PGV 9.9% at 5-7 yr, with more complications and 1 death after antrectomy).\n2. Sucralfate and cimetidine after stopping: RC-4557 cites Solhaug 1987 (PMID 3475770: cimetidine 59%, sucralfate 51%). RC-4554 cites Hallerbäck 1987 (PMID 3313677; same trial as Glise PMID 3310625: 71% vs 68%). These are reports of one Scandinavian trial at different stages (n=270 vs n=258 followed). Both mix duodenal, prepyloric and gastric ulcers. RC-4557 flags this in its table, although its claim sentence does not. RC-4554 does not flag it. The mixed-site figure should not be read as a DU relapse rate. This is why the DU-only H2RA range in my claim is 78-89% (Hamilton, Miller, Lee), not 51-90%.\n3. Surgery. RC-4554's claim of \"roughly 5-10% per decade\" after highly selective vagotomy is not supported. Its own sources give 30% at 14-18 yr (about 17-21% per decade; the result itself computes ~2%/yr) and 9.2% at 6-13 yr. Both verifiers of RC-4554 (codex and me) marked it partial for this reason. RC-4557's \"0-30% over 4-18 years\" is accurate as a range, but calling it \"low\" is generous at the upper end. Neither result has a 12-month surgical figure, and both rightly say the surgical figures cannot be compared with the drug rows.\n4. Maintenance: RC-4557's 35% vs 88% (Alstead, DU-only) and RC-4554's 16-43% (Silvis, DU, no placebo; Liedberg, mixed sites) are consistent. The overall range is 16-43% on active drug and 72-88% on placebo.\n\nWHICH HOLDS UP: Both tables reproduce against the abstracts. RC-4557's claim is closer to the data on surgery. RC-4554 covers the drug rows more fully, with a third bismuth-vs-H2RA trial and the maintenance sucralfate trial, but its surgery figure is wrong. The combined, corrected statement is the claim above. Neither is fully exact: RC-4557 presents the mixed-site Solhaug figure as a DU rate, and RC-4554 understates recurrence after vagotomy.",

All are dated 1982 to Nov 1987, so all fall within the cutoff.\n\nThe comparison's account of each result reproduces. Where RC-4554 and RC-4557 agree, they do. Its reading of the differences is correct:\n- Solhaug 1987 and Hallerbäck 1987 are the same Scandinavian group with the same design. Both mix duodenal, prepyloric and gastric ulcers.\n- RC-4554's \"5-10% per decade\" after vagotomy is not supported. Hoffmann gives 30% proven recurrence at 14-18 yr, and Herrington 9.2% at 6-13 yr.\n- Lam 1987 does report 50% sucralfate relapse at 12 months.\n\nMost of the claim sentence checks out:\n- H2RA stopped, DU-only trials, 78-89%: Hamilton 78%, Miller 85%, Lee 89%. Correct.\n- Bismuth 39-62%: Miller 39%, Hamilton 43%, Lee 62%. Correct.\n- Maintenance 16-43%: Silvis 16% and 43%, Alstead 35%, Liedberg 16% (mixed ulcer sites). Correct.\n- Coghlan 1987: n=39, 10% relapse if C. pylori stayed negative, 66% if it recurred, 79% if persistently culture-positive, observational. Correct.\n- Hoffmann: 30% at 14-18 yr. Correct.\n\nOne error in the claim: \"Recurrence after proximal gastric vagotomy was 0-20% at 4-13 years.\" The 0% is Donahue 1984's selective vagotomy plus antrectomy arm, not the PGV arm. The PGV figures in these sources are Donahue 20% (4-12 yr), De Vries 9.9% (5-7 yr) and Herrington 9.2% (6-13 yr). The claim should read \"about 9-20% at 4-13 years\" for PGV, with 0% only after vagotomy plus antrectomy.\n\nMinor points:\n- Hoffmann's cohort also included pyloric and prepyloric ulcers.\n- \"No randomised eradication-relapse trial was found\" is a finding of the search, not proof that none existed. The claim's wording is appropriately hedged.\n\nBecause of the PGV lower bound, the claim is not exactly supported, so my verdict is partial rather than confirmed.",

All are dated 1981-1987.\n\nThe detail reproduces:\n- Stopping after healing: cimetidine 85% vs CBS 39% (Miller 1982); ranitidine 89% vs TDB 62% (Lee 1985); cimetidine 78% vs TDB 43% (Hamilton 1986, duodenal ulcer).\n- Solhaug (59%/51%) and Hallerbäck/Glise (71%/68%) mix duodenal, prepyloric and gastric ulcers, as the detail says.\n- Maintenance: Alstead 1983 DU 7/20 vs 15/17; Silvis 16% vs 43%; Liedberg 16% vs 72%.\n- Coghlan 1987: 59%, 79%/27% and 66%/10%, n=39.\n- Surgery: Hoffmann 30% + 9% suspected at 14-18 yr; Herrington 9.2% at 6-13 yr; De Vries PGV 9.9% at 5-7 yr; Donahue PGV 8/40 = 20% at 4-12 yr.\n- The criticism of RC-4554's '5-10% per decade' is correct.\n\nOne error in the claim sentence: it says 'Recurrence after proximal gastric vagotomy was 0-20% at 4-13 years'. In Donahue 1984 (PMID 6385314) the 0% belongs to selective vagotomy with antrectomy (0/46), not to PGV; the detail itself attributes it correctly. Across the cited sources, PGV recurrence was about 9-20% at 4-13 years (9.2%, 9.9%, 20%; 12-33% by era in Donahue), rising to 30% at 14-18 years. The claim therefore understates the minimum recurrence after PGV, so it is not exact as stated. Every other figure in the claim matches the abstracts.",
