"detail": "METHOD AND SCOPE. Read the complete submitted details and claims of RC-4539 and RC-4541 through uain_projects and T-29 through uain_tasks. Independently retrieved primary PubMed XML abstracts for PMID 20942646 and 20951424 with NCBI efetch; read PMID 19072827 at https://pubmed.ncbi.nlm.nih.gov/19072827/. Recomputed both entire sample-size sensitivity tables in Python using statistics.NormalDist().inv_cdf, math.sqrt and math.ceil. This is a comparison of hypothetical designs, not observed efficacy or a complete re-verification of every source in either submission.\n\nAGREEMENT. Both specify treatment-naive non-cirrhotic GT1, randomized arms, an off-treatment SVR24 endpoint, resistance and safety monitoring, and hypothetical effect assumptions. Both recognize that short-term RNA suppression does not establish sustained response. Le Pogam (JID, 15 Nov 2010, PMID 20942646) supports 4-week RG7128+SOC activity and no detected resistance by the specified analyses; that is not proof resistance cannot arise with longer exposure. INFORM-1 (Lancet, 30 Oct 2010, PMID 20951424) supports only up to 13 days of dual-DAA exposure, with primary RNA change at day 14. Neither report establishes the proposed long-duration regimens' efficacy or safety. HCV796 (Hepatology, March 2009, PMID 19072827) reports abnormal liver tests in 8% beyond 8 weeks and suspension; this justifies vigilance, not an assumption that RG7128 has the same toxicity.\n\nDIFFERENT QUESTIONS. RC-4539 tests superiority of 12-week RG7128 add-on versus placebo, with PR48 in both groups; it isolates the incremental effect of RG7128 on a standard backbone. RC-4541 tests the combined contribution of peginterferon AND ribavirin to RG7128/danoprevir over 24 weeks; it cannot isolate interferon's contribution alone. Its comparison is internally coherent but its quadruple-drug control has assumed, unvalidated SVR. Non-inferiority between these two regimens cannot establish either is effective relative to standard care: both could perform poorly. Absolute SVR estimates would still be informative. The NI margin of 15 points has no demonstrated preserved-effect basis here and is suitable only as an explicitly exploratory screening choice.\n\nARITHMETIC. RC-4539: using its displayed two-proportion formula, ceiling n for control .38, .42, .46 and gains .10/.15/.20/.25 at powers .80/.90 yields respectively:\n.38: 384/513,172/230,97/130,62/82;\n.42: 390/522,174/232,97/130,62/82;\n.46: 392/524,173/231,96/128,60/80.\nEvery table cell reproduces. RC-4541: n=(z(.9)+z(.8))^2*[pA(1-pA)+pB(1-pB)]/(pA-pB+M)^2 gives 84.14677 ->85/arm at pA=pB=.70,M=.15. For p=.60/.70/.80 and M=.10/.15/.20 the rows are 217,97,55 /190,85,48 /145,65,37; true difference -.05 at M=.15 yields 220,198,157. These match. Its 95/arm follows ceiling(85/.90), allowing 10% attrition; literal 10% enlargement is ceiling(85*1.10)=94. Clarify that wording. These are approximate planning calculations, not exact power validation of CMH or Newcombe analyses.\n\nREPAIRS NEEDED IN BOTH. Their analysis populations exclude randomized participants receiving no dose, so they are modified ITT, despite ITT labels. Use all randomized participants or relabel precisely. RC-4539 equates SVR24 after last PR dose with fixed week72; those coincide only for full 48-week completers. Specify early-stop, rescue and intercurrent-treatment handling and whether the endpoint is actual-last-dose+24 weeks or the scheduled week72 assessment. RC-4541 specifies actual-last-dose timing and rescue as failure more clearly, but its NI conclusion must remain exploratory. Missing=failure does not itself guarantee preservation of planned power; RC-4539's 42%/62% assumptions must apply to the final analyzed randomized population including anticipated missing outcomes. RC-4539's statement that R1626 suppression did not become durable cure is too categorical if read literally: its own synopsis reports SVR in both arms; the unanswered issue is an attributable gain.\n\nWHICH HOLDS UP. Prefer RC-4539 as the project's single proposed next trial after the above endpoint/estimand repairs: its comparator and incremental question are better anchored and its exposure extrapolation is smaller (4 to12 weeks versus13 days to24 weeks of dual DAAs). This is a methodological judgment, not a clinical recommendation or demonstration of safety. Retain RC-4541 as an alternative exploratory IFN/RBV-omission question, conditional on staged longer-exposure safety assessment and an absolute-efficacy criterion or validated comparator. Neither design proves the other's hypothesis and they should not be merged into a single claimed result. Implementation would require clinical, regulatory and ethics review. No post-2010 evidence was used.",

I re-implemented both formulas in Python (statistics.NormalDist, ceiling).\n- RC-4539 superiority table: all 12 cells reproduce exactly (.38: 384/513, 172/230, 97/130, 62/82; .42: 390/522, 174/232, 97/130, 62/82; .46: 392/524, 173/231, 96/128, 60/80).\n- RC-4541 NI table: n=84.1468, which rounds up to 85. The rows 217/97/55, 190/85/48 and 145/65/37 reproduce, and so do the true-diff −.05 cells 220/198/157 and α=.05 →116.\n- RC-4545's point on the 10% wording is correct: ceil(85/0.9)=95, but ceil(85×1.1)=94.\n(3) SOURCES. I fetched PMIDs 20942646, 20951424 and 19072827 via NCBI efetch. All three predate 2011, and each supports what RC-4545 says:\n- Le Pogam (JID 15 Nov 2010, epub 13 Oct): 85 patients on RG7128 plus SOC for 4 weeks, with no resistance by NS5B sequencing or phenotype and no S282T.\n- INFORM-1 (Lancet 30 Oct 2010): up to 13 days of treatment, primary endpoint the RNA change at day 14, no SVR measured.\n- HCV796 (Hepatology Mar 2009): abnormal LFTs in 8% of patients treated beyond 8 weeks, and the trial was suspended.\n(4) REASONING. Each of the following is correct against the protocols:\n- RC-4539 isolates RG7128's added effect over PR48.\n- RC-4541 tests PR (PegIFN+RBV jointly) on top of the dual DAAs, with an assumed quad SVR of 70%. Non-inferiority to an unvalidated regimen therefore cannot establish efficacy versus standard of care.\n- Both use a modified-ITT population (≥1 dose) while calling it ITT.\n- RC-4539's \"week 72\" equals last dose + 24 weeks only for patients who complete the full course.\n- RC-4539's own numbers (R1626 58% vs 50%) make \"did not become durable cure\" too categorical.\nClaim is supported as stated. Caveat outside the claim: T-29 also has RC-4544 (T12PR48 vs T12PR24, submitted 36 s before RC-4545), and RC-4545 does not compare it. Its detail line calling RC-4539 \"the project's single proposed next trial\" therefore does not account for RC-4544. The claim sentence itself covers only RC-4539 vs RC-4541, so this does not affect the verdict. A full T-29 comparison including RC-4544 is still needed.",

"note": "What I checked:\n\n(1) I read the full details of RC-4539 and RC-4541 through uain_projects.\n\n(2) I recomputed both sample-size tables in my own Python, using statistics.NormalDist.\n- RC-4539's superiority table reproduces exactly: 0.38 → 384/513, 172/230, 97/130, 62/82; 0.42 → 390/522, 174/232, 97/130, 62/82; 0.46 → 392/524, 173/231, 96/128, 60/80.\n- RC-4541's non-inferiority table also reproduces: n = 84.1468 → 85; pB 0.6/0.7/0.8 gives 217/97/55 (220), 190/85/48 (198) and 145/65/37 (157).\n- ceil(85/0.9) = 95 but ceil(85 × 1.1) = 94, so RC-4545's wording point is correct.\n\n(3) I fetched abstracts through NCBI efetch.\n- PMID 20942646 (Le Pogam, JID 15 Nov 2010, epub Oct 2010): 4 weeks of RG7128 + SOC, with no resistance by NS5B sequencing or phenotype and no S282T.\n- PMID 20951424 (INFORM-1, Lancet 30 Oct 2010, epub 14 Oct): up to 13 days of dosing, primary endpoint the day-14 RNA change (-3.7 to -5.2 log10).\n- PMID 19072827 (HCV-796, Hepatology Mar 2009): abnormal LFTs in 8% of patients treated more than 8 weeks, and the trial was suspended.\nAll three are as RC-4545 states.\n\n(4) The reasoning.\n- The two designs do ask different questions: RG7128 add-on vs placebo on PR48, and PR added to or omitted from an RG7128 + danoprevir backbone.\n- RC-4539's comparator, PR48, was the 2010 standard of care.\n- RC-4541 randomises both arms to investigational DAAs, with pB = 0.70 an unvalidated assumption. Its own limitations section says \"no comparison with standard care\". So non-inferiority between its two arms cannot establish efficacy against standard care.\n- Both designs define their analysis population as randomised patients with at least one dose, which makes it modified ITT, as noted.\n- RC-4539 treats \"24 weeks after the last PR dose\" as the same as week 72, which is true only for patients who complete treatment, as noted.\n- The R1626 point is fair: RC-4539's own synopsis reports SVR of 58% vs 50%.\n- The exposure-extrapolation comparison (4→12 weeks vs 13 days→24 weeks) is correct.\n\nScope note: RC-4545 does not compare RC-4544, a third T-29 result submitted at about the same time. Its claim is explicitly limited to RC-4539 and RC-4541, so the claim is still accurate, but the T-29 comparison is not complete without RC-4544.",
