SVR24 was the primary endpoint in A1–A3. Swain 2010 (A4) showed that 99.1% of 1343 people with SVR stayed RNA-negative for a mean 3.9 years, which justified treating SVR24 as a proxy for durable cure *for interferon-based therapy*. Results by population:\n- Treatment-naive GT1: 42–52% SVR24 with 48 weeks.\n- GT2/3: about 80% SVR24, and 24 weeks was enough (A3).\n- Prior PegIFN/RBV nonresponders: 7–16% (A5).\n- Nonresponders with advanced fibrosis: 14–18% (A6, A7).\n- Relapsers did better than nonresponders (38% vs 14% in A7).\n\n**Most credible near-term improvement.** Adding an NS3/4A protease inhibitor to PegIFN/RBV for GT1. Four phase-2 RCTs measured SVR24 as the primary endpoint and compared against a concurrent PR48 control (B1–B4):\n- Treatment-naive: 38–46% SVR24 in the controls. Arms that beat control significantly reached 54–75%, and the best arm in each trial reached 67–75%.\n- Prior failures (PROVE3): 51–53% vs 14%.\n\nEach comparison is within a single trial, so no cross-trial inference is needed. Because the SVR24 endpoint is the same one validated as durable for interferon-based cure, these are measured off-treatment responses, not just suppression [inference: we assume SVR durability carries over to PI-containing regimens; there were no long-term PI follow-up data by 2010]. The limits:\n- The benefit depended on the interferon/ribavirin backbone. Arms without ribavirin did worse: T12P12 36%, T24P24 24%, and low-dose RBV gave 27% breakthrough.\n- Telaprevir alone quickly selected resistance (B5).\n- Harms were greater: rash and more discontinuations with telaprevir, anaemia and dysgeusia with boceprevir.\n- The evidence covered GT1 only and was phase 2. Phase-3 results had not been published in peer-reviewed journals by the cutoff.\n- For GT2/3, no PI data were reviewed that could change the ~80% baseline.\n\n**Longer-term hypotheses (no durable cure shown).**\n- Interferon-free DAA combinations: INFORM-1 (C2) showed about 5 log10 suppression over 13 days, including in null responders, but measured no SVR.\n- NS5A inhibition: a single dose only (D1), with fast resistance.\n- Cyclophilin inhibition: strong on-treatment declines (E1, E2), no SVR endpoint.\n- miR-122 antagonism: chimpanzees only (E5).\n- Therapeutic vaccines: immunogenic, but no clearance attributable to vaccination (F1, F2).\n- Polymerase inhibitors: the only durable endpoint (C1) was measured on an interferon backbone and showed no gain over SOC (24–58% vs 50%, small arms). High relapse was linked to R1626 being given for only 2–4 weeks and to neutropenia-driven interruptions.\n- Nitazoxanide is an outlier. It is the only host-targeting agent with a measured SVR, including without interferon (17.4%, E4). But the data come from two small single-country GT4 trials, and its mechanism was unclear, so this counts as low-maturity evidence, not a leading route [inference].\n\n**Why the ranking holds.** The ranking rests on evidence maturity: RCTs with SVR24 as the primary endpoint and concurrent controls come first, then short-term virology, then animal and immunogenicity data. It does not compare effect sizes across trials. IL28B (A8) was a known predictor of PR response by 2009, so baseline SVR rates in any trial depend on the host population mix [inference].\n\n**Bounded conclusion.** On evidence public by 2010-12-31, PegIFN/RBV was the only approach with phase-3, durability-supported cure (GT1 naive 42–52%, GT2/3 ~80%, prior nonresponders 7–18%). The strongest near-term improvement was telaprevir or boceprevir added to PegIFN/RBV in GT1, which raised SVR24 in phase-2 RCTs (naive: 54–75% in significant arms vs 38–46%; prior failures 51–53% vs 14%). Every interferon-free or non-PI novel route was still unproven for durable cure.\n\n## 3) Correction / uncertainty ledger\n- CL-25 (dispute upheld here): I use the full naive range of significant arms, 54–75%, not 61–75%. Resistance on monotherapy is stated for telaprevir only (Sarrazin 2007). I cite no boceprevir monotherapy resistance source.\n- CL-26: I do not claim that Gao 2010 was the \"only\" human NS5A evidence. I excluded the AASLD 2010 conference abstracts (1881, LB-8) because I could not retrieve their original dated content. I claim only that no reviewed primary report measured NS5A SVR.\n- CL-28: IC41 RNA responders are stated as 3 of **all 60 randomised patients**, not 3 of 60 IC41 recipients.\n- CL-29/33/34: R1626 week-72 SVR is given with all four arms and denominators, taken from the letter's Table 1 on natap.org (the publisher copy was not accessible). \"Only on-treatment\" is not applied to polymerase inhibitors as a class. HCV-796 longer-exposure and post-treatment anecdotes (PMID 19098685) were not re-opened by me and are not used.\n- CL-30/31: I do not state \"no host-targeting agent had interferon-free cure in a controlled trial\" (contradicted by E4). \"0 of 24 placebo\" is labelled on-treatment. The host-targeting list is \"reports reviewed\", not an exhaustive census.\n- Hadziyannis 2004 defines SVR as undetectable RNA \"during 12 to 24 weeks of follow-up\", so it is labelled differently from the strict SVR24 in the other trials.\n- Access limits: abstracts only for every source except the R1626 letter (natap transcription). Arm-level harms beyond the abstracts were not checked. Phase-3 PI trials (ADVANCE, SPRINT-2, RESPOND-2, REALIZE) were presented only as conference abstracts in late 2010 and were **excluded**, since later journal publication cannot supply historical results.\n- Swain 2010 durability covers PegIFN α-2a-based SVR only. Durability of PI-based SVR is an inference.\n- Swain 2010, HALT-C, EPIC3 and IL28B were not in T-22's contribution. I added them from my own search.",

REPEAT 16/7/14/9% and 72 vs 48 wk 16% vs 8%. HALT-C 18%, 35% at wk 20, RBV reduction 21%→11%. EPIC3 22% overall, relapsers 38% vs nonresponders 14%, prior PegIFN/RBV 17%. Ge IL28B about twofold.\n- Protease-inhibitor trials: PROVE1 41% (31/75), 61% (48/79), 67% (53/79), 35% (6/17), AE discontinuation 21% vs 11%, breakthrough 7%. PROVE2 46% (38/82), 69% (56/81), 60% (49/82), 36% (28/78). PROVE3 14/51/53/24%, arms 115/113/111/114, rash 51% (severe 5%), discontinuation 15% vs 4%. SPRINT-1 38% (39/104), 54% (58/107), 56% (58/103), 67% (69/103), 75% (77/103), low-dose RBV breakthrough 16/59, anaemia 55% vs 34%, dysgeusia 27% vs 9%. Sarrazin: resistant variants replaced by wild type within 3-7 months.\n- R1626: arms 21/32/31/20, wk-4 undetectable 29/69/74% vs 5%, grade 4 neutropenia 48/78/39/10%.\n- INFORM-1: −3.7 to −5.2 log10; −5.1 in naive and −4.9 in null responders vs +0.1 on placebo; no SVR.\n- BMS-790052: −3.3 log10 at 24 h, variants by 144 h.\n- Debio-025: −3.6 log10; −4.61/−4.75 and −5.91/−5.89 log10; hyperbilirubinaemia at 1000 mg.\n- Nitazoxanide: 79% vs 50% (p=.023) and 61%; monotherapy 7/23 vs 0/24 on treatment, SVR 4/23 (17.4%).\n- Lanford: chimpanzees only. Klade: 3 RNA responders \"within all 60 patients\". Wedemeyer: \"did not prevent HCV-RNA relapse\".\nThe ranking is by evidence maturity and uses within-trial comparisons only. The corrections from CL-25, CL-26, CL-28, CL-30, CL-31 and CL-33/34 are carried over correctly, and SVR24 is kept separate from on-treatment suppression throughout.\n\nWhy partial and not confirmed (the work reproduces, but the claim is inexact):\n(1) The claim's \"GT1 treatment-naive 42–52%\" range for PegIFN/RBV phase-3 evidence leaves out IDEAL (McHutchison, NEJM 2009-08-06, PMID 19625712). IDEAL is the largest pre-cutoff phase-3 RCT, with 3070 naive GT1 patients on 48 weeks of weight-based RBV, and its SVR was 39.8%, 38.0% and 40.9%. It is in RC-4526 and underlies the believed CL-27 (\"about 40–52%\"). The lower bound of 42% also comes from Manns' arm that used flat 800 mg RBV. The range supported by the evidence is about 38–52%. This is the same incomplete-range defect the task's how_to_check warns about (CL-25).\n(2) \"No interferon-free ... approach had shown durable cure\" sits uneasily with E4 in the same contribution: nitazoxanide monotherapy, which is interferon-free, gave SVR in 4/23. The carve-out that follows (\"nitazoxanide had only small single-country GT4 SVR data\") partly fixes this. A precise wording would be \"no interferon-free direct-acting-antiviral regimen had a measured SVR\".\n(3) Minor: the 7–18% range for prior nonresponders takes its 18% from HALT-C, an uncontrolled lead-in, although the sentence introduces it as phase-3 randomized evidence.\nSuggested restatement: \"…GT1 treatment-naive about 38–52% (IDEAL 38–41%; Hadziyannis 52%)…; no interferon-free DAA regimen had measured SVR; nitazoxanide had small single-country GT4 SVR data including 4/23 without interferon.\" With that, I would confirm.",

Almost every figure in the evidence matrix matches its source.\n- Manns 2001 (11583749): 54% (274/511), GT1 42% (145/348), GT2/3 about 80%.\n- Fried 2002 (12324553): 56/44/29%, GT1 46%.\n- Hadziyannis 2004 (14996676): 63%, GT1 52% (46–58), +11.2% for 48 vs 24 weeks, GT2/3 no significant difference.\n- Swain 2010: 99.1% of 1343 over a mean 3.9 years.\n- REPEAT (19380853): 16/7/14/9%, and 16% vs 8% by duration.\n- HALT-C (15057741): 18%, 35% undetectable at week 20, ribavirin reduction 21%→11%.\n- EPIC3 (19208349): 22% overall, relapsers 38% vs nonresponders 14%, prior PegIFN/RBV failures 17%.\n- IL28B (19684573): about twofold effect.\n- PROVE1 and PROVE2 (19403903: 46% (38/82), 69% (56/81) p=0.004, 60% p=0.12, 36%) and PROVE3: all as stated.\n- SPRINT-1 (20692693): 54/56/67/75% vs 38% (39/104), breakthrough 27% (16/59), anaemia 55% vs 34%, dysgeusia 27% vs 9%.\n- Sarrazin 2007: rapid selection; wild type returns within 3–7 months.\n- R1626 (18570306 and the natap letter): week-4 figures 29/69/74% vs 5%, neutropenia 48/78/39% vs 10%, relapse 6/11, 7/25 and 2/12.\n- INFORM-1 (20951424): −3.7 to −5.2 log10, −5.1 and −4.9 vs +0.1, no SVR.\n- Gao 2010: −3.3 log10 at 24 h.\n- Debio-025: −3.6 log10 (monotherapy study); −4.61/−4.75 (GT1/4) and −5.91/−5.89 (GT2/3) with PegIFN.\n- Nitazoxanide: triple therapy 79% vs 50% (p=0.023), NTZ+PegIFN 61%; monotherapy 7/23 on treatment vs 0/24, SVR 4/23 (17.4%).\n- miR-122: chimpanzees only.\n- IC41: 3 RNA responders among all 60 patients; the add-on study did not prevent relapse.\nThe pooled naive ranges also check out. Significant PI arms were 54, 56, 61, 67, 67, 69 and 75%, giving 54–75%. Controls were 38, 41 and 46%, giving 38–46%. Prior failures were 51–53% vs 14%. The synthesis and its ranking by evidence maturity reproduce.\n\nWhy partial and not confirmed: two parts of the stated claim are wider than the evidence.\n(1) \"Prior nonresponders 7–18%\" sits under \"phase-3 randomized SVR24 evidence\". Only REPEAT is randomized, and it gives 7–16% by arm. The 18% comes from HALT-C, a non-randomized lead-in in prior interferon-based nonresponders (including IFN monotherapy), not PegIFN/RBV nonresponders.\n(2) \"No interferon-free … approach had shown durable cure\" conflicts with the author's own E4. Nitazoxanide monotherapy was interferon-free and measured SVR24 in 4/23 treated patients in a randomized trial. The next clause mentions nitazoxanide SVR data, but only as \"GT4 SVR data\", without saying it was interferon-free. As written, the sentence can be read as contradicting the detail and the CL-30/31/39 corrections.\nSuggested exact wording: \"prior PegIFN/RBV nonresponders 7–16% in the randomized REPEAT trial (14–18% in non-randomized fibrosis cohorts)\" and \"apart from one small single-country GT4 nitazoxanide monotherapy trial (SVR 4/23), no interferon-free … approach had measured SVR\". Note that RC-4542 has the same claim text; the author says RC-4543 supersedes it.",
