"claim_statement": "A double-blind phase-2 trial justified by pre-2011 evidence would randomize 200 treatment-naive, non-cirrhotic genotype-1 adults 1:1 to RG7128 1000 mg bid or placebo for weeks 0-12, both arms on fixed 48-week PegIFN alfa-2a/RBV, with ITT SVR24 (undetectable HCV RNA at week 72) as the primary endpoint; this gives about 80% power (two-sided alpha 0.05) to detect a hypothetical rise from 42% to 62%, and tests whether RG7128's resistance-free on-treatment suppression becomes durable cure, which R1626's early suppression did not.",

"detail": "EVIDENCE TYPE: proposed. Hypothetical study design only. No outcome here was observed. It is not a clinical instruction, and it would need full clinical, regulatory and ethics review (IRB/EC, DSMB charter) before it could be run. Every source below was published by 2010-12-31. I fetched each PubMed record on 2026-09-25 with NCBI efetch.\n\n1. HYPOTHESIS AND SOURCE-LINKED GAP\nObserved:\n(a) Nucleoside NS5B inhibitor RG7128 given with PegIFN/RBV for 4 weeks produced mean falls of up to 5 log10. Among 85 combination-treated patients, no resistance was found by NS5B population/clonal sequencing or phenotyping, and S282T was not detected. Source: Le Pogam, J Infect Dis 2010;202:1510-9, epub 13 Oct 2010, PMID 20942646, abstract Introduction/Results.\n(b) The only polymerase regimen with measured SVR24 by 2010 was R1626. R1626 TRIPLE 1500 gave undetectable HCV RNA at week 4 in 74%, versus 5% with SOC (Pockros, Hepatology 2008;48:385-97, PMID 18570306, abstract Results). Yet week-72 SVR was 18/31 (58%) versus 10/20 (50%) with SOC, driven by high relapse (Pockros letter, Hepatology 2008;48:1349-50, PMID 18821613, Table 1; figures as checked in believed CL-32/RC-4533). R1626 also caused grade-4 neutropenia in 39-78% of patients (PMID 18570306).\n(c) Polymerase class toxicity: HCV-796 caused abnormal liver tests in 8% of patients treated for more than 8 weeks, and its trial was suspended (Kneteman, Hepatology 2009;49:745-52, PMID 19072827).\n(d) INFORM-1 (Gane, Lancet 2010;376:1467-75, epub 14 Oct 2010, PMID 20951424) tested RG7128 with danoprevir for only 13 days. The endpoint was day-14 change (median -3.7 to -5.2 log10), with no SVR measured.\nGap (inference): no pre-2011 report measured SVR24 for any RG7128 regimen. The R1626 precedent shows that deep week-4 suppression need not produce a durable SVR gain. Whether a nucleoside with a high resistance barrier and no reported neutropenia adds off-treatment cure to PegIFN/RBV is therefore unanswered. RC-4533, RC-4535, RC-4536 and CL-33/35 agree that no attributable polymerase SVR benefit had been shown.\nHypothesis: SVR24 is higher with RG7128 plus PR than with placebo plus PR.\n\n2. POPULATION\nAdults aged 18-65 with chronic HCV genotype 1 (1a or 1b), treatment-naive, HCV RNA ≥50,000 IU/mL and compensated liver disease, with METAVIR F0-F3 on biopsy within 24 months.\nExcluded: cirrhosis, HIV or HBV co-infection, other liver disease, neutrophils <1500/µL, Hb <12 g/dL (women) or <13 g/dL (men), platelets <90,000/µL, uncontrolled psychiatric disease, pregnancy, and inability to use two contraceptive methods (ribavirin teratogenicity).\nWhy narrow: prior failures, cirrhosis and genotypes 2/3 have different baselines (CL-27: about 80% SVR in GT2/3; 7-16% on PR re-treatment of nonresponders). Mixing them would dilute the estimand.\n\n3. RANDOMIZED ARMS (one comparison, 1:1, double-blind)\n- Arm A: RG7128 1000 mg orally bid for weeks 0-12, plus PegIFN alfa-2a 180 µg/week and weight-based RBV (1000 mg if <75 kg, 1200 mg if ≥75 kg) for 48 weeks.\n- Arm B: matched placebo for weeks 0-12, plus the same PR for 48 weeks.\nTotal PR duration is fixed at 48 weeks in both arms, with no response-guided shortening. Any SVR difference is then attributable to RG7128, not to duration. The 1000 mg bid dose is one of the doses with 4-week PR data in PMID 20942646.\nRandomization: central, computer-generated permuted blocks, stratified by IL28B rs12979860 (CC vs non-CC; Ge, Nature 2009;461:399-401, PMID 19684573, which reports an approximately twofold effect on SVR) and by baseline HCV RNA (<800,000 vs ≥800,000 IU/mL).\nComparator rationale: in 2010, PR48 was the standard of care and the control in PROVE1 (PMID 19403902) and SPRINT-1 (PMID 20692693). Telaprevir and boceprevir were still investigational and unapproved.\nEquipoise: RG7128 SVR data do not exist, and R1626 shows early suppression can fail to translate. Both arms receive a regimen with established curative potential.\n\n4. ESTIMANDS AND ENDPOINTS\nPrimary: SVR24, defined as HCV RNA below the limit of detection (<15 IU/mL, real-time PCR) 24 weeks after the last PR dose, i.e. at week 72, in all randomized patients who received at least one dose. The time origin is end of treatment, not randomization, which follows the SVR definitions of PMID 12324553, 19403902 and 20692693.\nSecondary:\n- relapse (undetectable at end of treatment, then detectable during follow-up);\n- viral breakthrough;\n- RVR (week 4) and cEVR (week 12), which are on-treatment markers only and not surrogates for cure;\n- SVR12;\n- week-96 durability (SVR48) in SVR24 responders;\n- adverse events, discontinuations, grade 3-4 neutropenia, anemia, ALT elevation;\n- results by IL28B and HCV subtype 1a/1b (exploratory).\nHCV RNA sampling: weeks 0, 1, 2, 4, 8, 12, 24, 36, 48 (end of treatment), and follow-up weeks 4, 12, 24 and 48. This lets the analysis separate on-treatment suppression from off-treatment relapse.\n\n5. ANALYSIS AND MISSING DATA\n- Primary comparison: stratified Cochran-Mantel-Haenszel test at two-sided alpha 0.05, reporting the risk difference with 95% CI.\n- Missing week-72 HCV RNA counts as failure (non-responder imputation).\n- Sensitivity analyses: (i) SVR imputed if the week-72 sample is missing but the next available sample is undetectable; (ii) multiple imputation; (iii) per-protocol.\n- No interim efficacy analysis. The DSMB reviews unblinded safety only.\n\n6. SAMPLE SIZE\nFormula (two proportions, normal approximation): n per arm = [z(1-α/2)·√(2p̄(1-p̄)) + z(1-β)·√(p1(1-p1)+p2(1-p2))]² / (p2-p1)².\nInputs:\n- p1 = 0.42. This is an observed-range assumption, the midpoint of the pre-2011 PR48 control SVRs: 38% SPRINT-1, 41% PROVE1, 46% PROVE2.\n- p2 = 0.62. This effect is hypothetical.\n- α = 0.05 two-sided, power 80%.\nResult: 97 per arm. Planned enrolment is 100 per arm, 200 in total. Non-responder imputation already counts dropouts as failures, so no further inflation is applied.\nSensitivity (n per arm at 80% / 90% power; computed with Python statistics.NormalDist, script ss.py):\n| control | +10 pts | +15 pts | +20 pts | +25 pts |\n|---|---|---|---|---|\n| 0.38 | 384 / 513 | 172 / 230 | 97 / 130 | 62 / 82 |\n| 0.42 | 390 / 522 | 174 / 232 | 97 / 130 | 62 / 82 |\n| 0.46 | 392 / 524 | 173 / 231 | 96 / 128 | 60 / 80 |\nWith 100 per arm, only effects of about 20 points or more are reliably detectable. A true gain of 10 points (similar to R1626's 58% vs 50%) would be missed, which the limitations section flags.\n\n7. SAFETY, RESISTANCE, STOPPING AND RESCUE\nSafety:\n- CBC, ALT/AST and bilirubin at weeks 1, 2, 4, then every 4 weeks.\n- Pre-specified RG7128 stopping rules drawn from class signals: ALT >5×ULN or >3×ULN with bilirubin >2×ULN (HCV-796 hepatotoxicity, PMID 19072827), and neutrophils <500/µL not reversed by PegIFN dose reduction (R1626, PMID 18570306).\n- PR dose modifications for anemia and neutropenia follow label rules.\n- Rash, psychiatric and ocular AE monitoring.\nFutility (both arms): stop all therapy if HCV RNA falls less than 2 log10 by week 12 or is detectable at week 24.\nResistance: NS5B population sequencing, plus clonal sequencing for S282T, at baseline, at virologic breakthrough (≥1 log10 rise from nadir, or >100 IU/mL after being undetectable), at relapse, and at the end of follow-up.\nDSMB: may pause RG7128 dosing if grade-4 hepatic or hematologic events differ between arms.\nRescue: failures are unblinded after week 72, or at futility stop, and referred for available standard or trial therapy. No patient continues RG7128 without PR.\n\n8. HOW T-28 CONCLUSIONS AFFECT THE RATIONALE\n- If T-28 concludes that protease-inhibitor triple therapy (CL-25 range: 54-75% in naive GT1) is the strongest near-term route, this trial still addresses a different open question: the polymerase class with a high resistance barrier. A later design could make PI triple therapy the comparator once approved.\n- If T-28 instead ranks interferon-free DAA combinations (INFORM-1) as most promising, this trial matters less. The natural alternative would be an SVR24 study of RG7128 plus danoprevir, but that has no pre-2011 SVR baseline and carries a higher resistance risk (danoprevir is a protease inhibitor, and telaprevir alone rapidly selects resistance: PMID 17484874).\n- Host or vaccine routes (CL-24, CL-28) have no human SVR benefit and would not change this design.\n\n9. FEASIBILITY AND LIMITATIONS\n- 200 patients is feasible for a multicentre phase 2 trial, comparable to PROVE1 arms.\n- RG7128 exposure beyond 4 weeks is untested, so 12-week safety is itself uncertain.\n- The 42% baseline may not transfer across populations with different IL28B distributions, which is handled only partly by stratification.\n- 48-week PR carries a large toxicity burden in both arms.\n- The trial is underpowered for gains under 20 points.\n- It cannot address interferon-free cure, cirrhosis, or prior nonresponders.",

"note": "Evidence type is proposed, correctly labelled; all outcomes are marked hypothetical and the text says clinical/ethics review is needed. (1) Sample size: I re-implemented the stated two-proportion normal-approximation formula in Python (statistics.NormalDist). All 12 sensitivity cells match exactly (for example 0.42 vs 0.62 gives 97/130 per arm at 80%/90% power; 0.42 +10 pts gives 390/522). Exact power at 100/arm for 42% vs 62% is 81.3%, so \"about 80% power\" holds. (2) Sources: I fetched all 10 PubMed records via NCBI efetch. All were published by 2010-12-31 and match their stated content. PMID 20942646 (Le Pogam, JID, epub 13 Oct 2010): 85 combination patients, no resistance by NS5B sequencing or phenotyping, no S282T, up to 5 log10 at 1000/1500 mg bid + SOC for 4 wk. PMID 20951424 (INFORM-1, epub 14 Oct 2010): primary endpoint is the day-14 change, median -3.7 to -5.2 log10, no SVR. PMID 18570306 (R1626): week-4 undetectable in 74% TRIPLE 1500 vs 5% SOC; grade-4 neutropenia 39-78% in R1626 arms. PMID 19072827 (HCV-796): abnormal LFTs in 8% after >8 wk, trial suspended. PMID 19403902 (PROVE1): PR48 SVR 41% (31/75), placebo for weeks 0-12, a double-blind design analogous to this one. PMID 20692693 (SPRINT-1): PR48 38% (39/104). PMID 12324553: SVR defined 24 wk after end of therapy. PMID 19684573: IL28B has about a twofold effect. PMID 17484874: telaprevir rapidly selects resistance. PMID 18821613 is a letter with no abstract, so I could not check the 18/31 vs 10/20 week-72 figures directly; the contribution relies on believed CL-32/RC-4533 for them. The endpoint timing is right: 48 wk of PR + 24 wk = week 72. The follow-up schedule (FU wk 4/12/24/48, relapse as a separate endpoint) separates relapse from on-treatment response. Stopping rules come from the cited class signals. Minor caveats that do not change the claim: (a) the PROVE2 46% control input has no source in the list (it would be Hezode, NEJM 2009, PMID 19403903); without it the 38%/41% sourced controls still support p1 of about 0.42, and the sensitivity table covers 0.38-0.46. (b) The primary population is \"randomized and received at least one dose\", which is modified ITT, while the claim says ITT. (c) The gap statement (\"no pre-2011 RG7128 SVR24\") holds for peer-reviewed reports I can find; phase-2 RG7128+PR trials were under way in 2010, but I found no published SVR24 before the cutoff. The claim matches the protocol and the arithmetic.",

Safety stopping rules map to the cited class signals. Hypothetical outcomes are labelled. The gap (no pre-2011 RG7128 SVR data) is consistent with the cited reports. Minor caveats that do not change the verdict: (a) the primary population is \"randomized and received ≥1 dose\", i.e. modified ITT, while the claim says ITT; (b) the PROVE2 46% control rate used for p1 is correct as I understand it but is not among the listed sources; (c) SPRINT-1 used PegIFN alfa-2b, not alfa-2a.
