"claim_statement": "By 2010-12-31, primary reports of HCV NS5B polymerase inhibitors (R1626, RG7128, HCV-796, and RG7128 plus danoprevir in INFORM-1) showed only on-treatment HCV RNA suppression over 13 days to 4 weeks (median fall of up to about 5 log10 IU/mL for the all-oral combination), without resistance on the nucleoside drugs and with dose-limiting toxicity for R1626 and HCV-796; none measured a sustained virologic response attributable to a polymerase inhibitor or to an interferon-free regimen.",

"detail": "METHOD (reproducible). PubMed E-utilities esearch, run 2026-09-25, with term \"(<drug>) AND hepatitis AND 1900:2010[pdat]\" for <drug> = R7128, R1626, HCV-796, \"filibuvir OR PF-00868554\", \"valopicitabine OR NM283\" (RG7128 alone returned nothing separate). I checked the hits by title and publication type and kept clinical primary reports, plus one preclinical resistance paper for mechanism. I read the full abstracts via efetch; the full texts are paywalled (Wiley 403, LWW 402), so every fact below is from the ABSTRACT unless marked otherwise. Excluded: filibuvir and valopicitabine had no primary clinical paper indexed by 2010 in this search (only reviews, preclinical or chemistry papers, e.g. PMID 19307358, 17064080). Dates come from PubMed pubdate and epubdate.\n\nMECHANISM (from PMID 18285474 abstract and the 19072827 title): nucleoside analogues (R1479/R1626, PSI-6130/RG7128, NM107/NM283) act at the NS5B active site. HCV-796 is a non-nucleoside (allosteric) NS5B inhibitor.\n\nEVIDENCE TABLE\n1) Roberts SK et al., Hepatology 2008;48(2):398-406. DOI 10.1002/hep.22321, PMID 18553458. Print date Aug 2008.\nPopulation: 47 treatment-naive patients with genotype 1. Design: phase 1b, observer-blinded, randomised against placebo, multiple ascending doses. Regimen: R1626 500, 1500, 3000 or 4500 mg twice daily, or placebo, for 14 days, then 14 days of follow-up.\nResult: mean HCV RNA fall at day 14 was 0.32, 1.2, 2.6 and 3.7 log10 respectively (medians 0.22, 0.8, 2.7, 4.1). No resistance to R1479 at 14 days.\nHarms: more adverse events at 4500 mg; reversible haematological changes that increased with dose. Endpoint was on-treatment only; SVR not measured. Per-arm n is not in the abstract.\n\n2) Pockros PJ et al., Hepatology 2008;48(2):385-97. DOI 10.1002/hep.22357, PMID 18570306. Aug 2008.\nPopulation: 104 treatment-naive patients with genotype 1. Design: phase 2a, randomised. Arms for 4 weeks: DUAL1500 (R1626 + peginterferon alfa-2a, n=21), DUAL3000 (n=32), TRIPLE1500 (R1626 + peginterferon + ribavirin, n=31), and standard of care (peginterferon + ribavirin, n=20).\nResult: HCV RNA below 15 IU/mL at week 4 in 29%, 69%, 74% and 5% (about 6/21, 22/32, 23/31 and 1/20; these counts are my recomputation). Mean week-4 falls were 3.6, 4.5, 5.2 and 2.4 log10. No resistance seen.\nHarms: grade 4 neutropenia in 48%, 78%, 39% and 10%; 7 patients had serious adverse events.\n3) Pockros P et al., letter, Hepatology 2008;48(4):1349-50. DOI 10.1002/hep.22593, PMID 18821613. Oct 2008. FULL TEXT NOT ACCESSED (paywalled).\nIts title reports a \"high relapse rate seen at week 72\". The only figures I have come from a SECONDARY review (Pockros, Ther Adv Gastroenterol 2010, PMC3002578, first published 2010-05-01). It gives, for the four arms in order, end-of-treatment response of 52, 66, 81 and 60%, and SVR of 24, 53, 58 and 50%.\nInference: these are post-treatment outcomes after the 4-week R1626 phase was followed by peginterferon and ribavirin. The TRIPLE arm (58%) did not clearly beat standard of care (50%; n=31 vs 20). That same review says R1626 was discontinued after a phase 2b showed severe lymphopenia (1 death) and visual impairment (8 patients).\n\n4) Kneteman NM et al., Hepatology 2009;49(3):745-52. DOI 10.1002/hep.22717, PMID 19072827. Mar 2009.\nNon-nucleoside HCV-796. Preclinical: EC50 of 5 nM (genotype 1a replicon) and 9 nM (1b); in chimeric mice, a 2.02±0.55 log fall on HCV-796 alone and 2.44 log with interferon. Clinical (treatment-naive): \"additive efficacy\" with peginterferon and ribavirin. Abnormal liver tests in 8% of patients treated beyond 8 weeks led to suspension of the trial.\nThe abstract gives no SVR and no clinical denominators. The McCown abstract (18285474) states that 14-day HCV-796 monotherapy \"rapidly selected for drug-resistant variants\".\n\n5) Le Pogam S et al., J Infect Dis 2010;202(10):1510-9. DOI 10.1086/656774, PMID 20942646. Epub 2010-10-13.\nRG7128 (prodrug of PSI-6130), genotypes 1, 2 and 3. 32 patients had 2 weeks of monotherapy and 85 had 4 weeks with standard of care. Mean falls: 2.7 log10 on monotherapy (1500 mg twice daily, 2 weeks) and 5 log10 with standard of care (1000 or 1500 mg twice daily, 4 weeks).\nViral rebound was marginal in 3/32 on monotherapy and occurred in 1/85 (plus 2 transient) with standard of care; 5 genotype-1 patients were still above 1000 IU/mL at week 4. No resistance by population or clonal NS5B sequencing and phenotyping; S282T was not detected. SVR not reported.\n\n6) Gane EJ et al. (INFORM-1), Lancet 2010;376:1467-75. DOI 10.1016/S0140-6736(10)61384-0, PMID 20951424, NCT00801255. Epub 2010-10-14.\nFirst published all-oral combination of a polymerase inhibitor and a protease inhibitor: RG7128 500 or 1000 mg twice daily plus danoprevir (100 or 200 mg every 8 h, or 600 or 900 mg twice daily) for up to 13 days, against placebo. Genotype 1, New Zealand and Australia; 88 randomised (74 to active drug, 14 to placebo). Included treatment-naive patients and prior null responders.\nPrimary endpoint was the change in HCV RNA at day 14. Median fall across cohorts was 3.7 to 5.2 log10. At the top dose it was -5.1 (IQR -5.6 to -4.7) in naive patients and -4.9 (-5.2 to -4.5) in null responders, versus +0.1 on placebo.\nNo treatment-related serious or severe adverse events, no grade 3/4 laboratory changes, no safety discontinuations. Resistance and SVR are not reported in the abstract. The 2010 secondary review says patients went on to peginterferon and ribavirin afterwards, so any later SVR could not be attributed to the oral combination.\n\nCONCLUSION (<=250 words)\nObserved: by the end of 2010, polymerase inhibitors had shown dose-dependent HCV RNA suppression in people. The nucleoside drugs (R1626, RG7128) showed no resistance over 2 to 4 weeks in these studies, in line with replicon data showing a higher resistance barrier (18285474). The non-nucleoside HCV-796 selected resistance quickly. INFORM-1 showed that a nucleoside polymerase inhibitor plus a protease inhibitor, with no interferon, reduced HCV RNA by a median of about 5 log10 in 13 days, including in null responders, with good short-term tolerability.\nToxicity stopped two candidates: R1626 (neutropenia, then lymphopenia and visual toxicity) and HCV-796 (liver enzyme elevations).\nNot measured: no report here gives an SVR (undetectable HCV RNA at least 24 weeks after treatment) for a polymerase-inhibitor-containing or interferon-free regimen that can be attributed to the polymerase inhibitor. The only SVR figures, for R1626 and from a secondary source, follow 44 weeks of peginterferon and ribavirin and do not beat the control arm.\nInference, not observation: whether interferon-free polymerase-based combinations could cure HCV was untested in 2010. A 5-log fall over 13 days is not evidence of cure.\n\nUNKNOWNS: the primary figures of the R1626 week-72 letter; INFORM-1 resistance and rebound data in the full text; clinical denominators for HCV-796 (the phase 2 appeared only as a 2009 EASL abstract); duration and relapse for any interferon-free regimen; genotypes other than 1 (RG7128 only). Conference abstracts from 2009-2010 (e.g. filibuvir, GS-9190) were not systematically searched.",

"note": "Re-ran it independently. I used PubMed efetch abstracts for PMIDs 18553458, 18570306, 20942646, 20951424, 19072827 and 18285474, and esummary for their dates. All are dated 2008-2010, and the two Oct 2010 papers have epubs of 13-14 Oct 2010, so all fall before 2010-12-31.\n\nChecked against the abstracts:\n- R1626 monotherapy mean day-14 falls of 0.32, 1.2, 2.6 and 3.7 log10, with no resistance.\n- Pockros arms 21/32/31/20; week-4 undetectable 29/69/74/5%; my recomputation matches: 6/21, 22/32, 23/31, 1/20.\n- Pockros grade 4 neutropenia 48/78/39/10%.\n- Le Pogam: 32 monotherapy and 85 combination patients; no resistance; S282T needs ≥90% of the quasispecies.\n- INFORM-1: 88 randomised (74 active, 14 placebo); median -3.7 to -5.2 log10; -5.1 in naive and -4.9 in null responders vs +0.1 on placebo; no serious or severe AEs.\n- HCV-796: EC50 5/9 nM; mice -2.02 log; abnormal liver tests in 8% beyond 8 weeks led to trial suspension.\n- McCown 18285474 abstract: HCV-796 monotherapy \"rapidly selected for drug-resistant variants\" after 14 days.\n\nOn the R1626 week-72 letter (PMID 18821613): the contributor used secondary figures. I checked them against the letter's text as reproduced at https://www.natap.org/2008/HCV/100908_02.htm.\n- The letter confirms that all patients received PEG-IFN/RBV for a further 44 weeks after week 4.\n- It gives TRIPLE end-of-treatment response 81%, the other experimental arms 52-66%, and the following relapses: SOC 2/12, DUAL1500 6/11, TRIPLE 7/25.\n- These counts reproduce the stated SVRs exactly: SOC 10/20 = 50%, DUAL1500 5/21 = 24%, TRIPLE 18/31 = 58%.\n- DUAL3000 SVR 53% is consistent (17/32) but its relapse count is not in the letter's text.\nSo SVR was measured, but only after 44 weeks of PEG/RBV, and it did not beat SOC. The claim's \"none measured an SVR attributable to a polymerase inhibitor\" therefore holds. Its \"only on-treatment suppression\" should be read in that sense, and the detail discloses this openly.\n\nNot independently checked: the review's statements on R1626 lymphopenia, death and visual toxicity, and on INFORM-1 patients continuing to PEG/RBV. These are secondary, labelled as such, and not in the claim.",

I checked them against the letter's Table 1 image reproduced at natap.org/2008/images/100908/End-1.gif (the letter is Hepatology 2008;48:1349-50). SVR at week 72 was 24% (5/21), 53% (17/32), 58% (18/31) and 50% (10/20); EOT was 52/66/81/60%. This matches the contribution exactly.\n\nThe claim's final clause is \"none measured an SVR attributable to a polymerase inhibitor or to an interferon-free regimen\". It is worded carefully, and I find it accurate: SVR WAS measured for the R1626 arms, but after 44 weeks of PEG-IFN/RBV, and no arm clearly exceeded SOC's 50%. No interferon-free regimen had any post-treatment endpoint.\n\nMinor wording notes, which do not change my verdict:\n- \"Dose-limiting toxicity\" for HCV-796 is really treatment-stopping hepatotoxicity that appeared after more than 8 weeks of combination therapy. That combination duration also lies outside the \"13 days to 4 weeks\" window.\n- The nucleoside \"no resistance\" finding covers only 2 to 4 weeks of exposure.",
