"claim_statement": "By 31 Dec 2010, primary reports on HCV polymerase inhibitors showed only on-treatment HCV RNA declines over 5 days to 4 weeks. These were up to 3.7 log10 mean with 14 days of R1626 alone, 5.2 log10 mean at week 4 with R1626 plus PEG-IFN/RBV, and a median of -3.7 to -5.2 log10 at day 14 with the interferon-free RG7128+danoprevir combination (INFORM-1). No treatment-emergent resistance was detected in these short studies. Toxicity was dose-limiting for R1626 (grade 4 neutropenia 39–78%), HCV-796 (abnormal liver tests in 8% beyond 8 weeks; trial suspended) and BILB 1941 (GI intolerance). None of the reviewed papers reported an SVR endpoint for a polymerase-inhibitor regimen.",

PubMed E-utilities esearch run on 2026-09-25, restricted to publication date (pdat) 1990 to 2010/12/31:\n(a) \"R1626 AND hepatitis C\" gave 10 hits.\n(b) \"RG7128 OR R7128 OR INFORM-1\" gave 12 hits.\n(c) \"(HCV-796 OR BILB 1941 OR filibuvir OR valopicitabine OR NM283) AND hepatitis C\" gave 32 hits.\nI read the esummary records. I excluded reviews, chemistry, replicon-only and PK papers. The human clinical primary reports that remained are listed below. I read the PubMed abstracts through efetch; full texts were paywalled, so this is an ABSTRACT-LEVEL review, and page/table locations are abstract sections. The search terms did not surface a primary clinical report of valopicitabine/NM283 or filibuvir published by 2010 (only reviews: PMID 17328231, 21154154), so they are excluded. On monotherapy these were up to 3.7 log10 mean at 14 days.\n- Added to PEG-IFN±RBV for 4 weeks, they gave week-4 undetectability in up to 74% (23/31) vs 5% on SOC.\n- The first interferon-free polymerase+protease combination (INFORM-1, Oct 2010) gave median declines of -3.7 to -5.2 log10 by day 14, including in null responders, and was well tolerated over 13 days.\n- The nucleosides showed no detectable resistance in these short courses.\n- Non-nucleosides (HCV-796, BILB 1941) were limited by hepatotoxicity or GI intolerance, and BILB 1941 was only weakly active.\n- R1626 caused dose-limiting neutropenia.\nInference:\n- The data showed potent short-term suppression and a plausibly high resistance barrier for nucleosides. They did not show cure.\n- None of the reviewed papers measured SVR (undetectable RNA 24 weeks after stopping) for an interferon-free polymerase-inhibitor regimen.\n- The one post-treatment report found (R1626 letter, Oct 2008) is titled as showing a high relapse rate at week 72.\n- Extrapolating day-14 or week-4 declines to cure would be hindsight.\n\nUNKNOWNS\n- The SVR and relapse numbers in the R1626 letter (not accessed).\n- Full-text tables for all papers.\n- Clinical HCV-796 efficacy magnitudes.\n- Durability of any response after stopping the INFORM-1 drugs.\n- Conference-only data (e.g. AASLD/EASL 2010 abstracts) are excluded.\nNo patient recommendation is made.",

"note": "I re-ran this from the PubMed abstracts (efetch) of 18553458, 18570306, 19072827, 20942646, 20951424 and 19320234.\n\nNumbers that reproduce exactly:\n- R1626 alone: mean fall of 3.7 log10 at 14 days (4500 mg twice daily).\n- R1626 + peginterferon/ribavirin (TRIPLE): mean fall of 5.2 log10 at week 4.\n- Grade 4 neutropenia: 48%, 78%, 39% and 10% (arms n=21, 32, 31, 20). The claim's range of 39–78% is correct for the R1626 arms.\n- INFORM-1: median day-14 change of -3.7 to -5.2 log10.\n- HCV-796: abnormal liver tests in 8% of patients treated beyond 8 weeks; trial suspended.\n- BILB 1941 (Antivir Ther 2009;14:23-32): 5-day dosing; all 5 patients on active 450 mg discontinued (4 for GI intolerance); trial stopped; no on-treatment resistance.\nAll publication dates are before 2011.\n\nProblems with the claim as worded:\n1) \"None of the reviewed papers reported an SVR endpoint for a polymerase-inhibitor regimen\" is contradicted by a source the contribution itself cites. PMID 18821613 (Pockros et al., Hepatology 2008;48:1349-50, October 2008) is titled \"High relapse rate seen at week 72…\". The letter's text, reproduced at natap.org/2008/HCV/100908_02.htm, reports week-72 outcomes (24 weeks after treatment) for the R1626 arms: relapse in DUAL1500 6/11 (55%), TRIPLE1500 7/25 (28%) and standard of care 2/12 (17%). A 2010 review (PMC3002578) gives SVR of 24%, 53%, 58% and 50% for DUAL1500, DUAL3000, TRIPLE1500 and standard of care.\nSo an SVR endpoint was measured by 2010 for regimens that contained a polymerase inhibitor (4 weeks of R1626, then peginterferon/ribavirin). What was never measured is an SVR attributable to the polymerase inhibitor, or any SVR for an interferon-free regimen. The claim needs that narrower wording.\n2) \"No treatment-emergent resistance was detected in these short studies\" is accurate for the abstracts of the nucleoside studies and BILB 1941. The HCV-796 abstract (19072827) does not address resistance. However, a pre-2011 primary source (McCown 2008, AAC, PMID 18285474) states that 14-day clinical HCV-796 monotherapy \"rapidly selected for drug-resistant variants\". The blanket statement therefore gives the wrong picture for the non-nucleoside class.\n\nThe work reproduces; the claim needs correcting on point 1 before it becomes project belief.",

"note": "I fetched all 7 PubMed records cited, using NCBI efetch abstracts on 2026-09-25. Most figures reproduce exactly:\n- R1626 14-day monotherapy mean declines were 0.32/1.2/2.6/3.7 log10 (PMID 18553458).\n- R1626 plus PEG-IFN±RBV at week 4 gave 3.6/4.5/5.2 log10, with grade 4 neutropenia of 48%/78%/39% (range 39–78%) (PMID 18570306).\n- HCV-796: abnormal liver tests in 8% of patients treated for more than 8 weeks, and the trial was suspended (PMID 19072827).\n- BILB 1941: 5 days of dosing, with GI intolerance at 450 mg (PMID 19320234).\n- RG7128: no resistance (PMID 20942646).\n- INFORM-1: median day-14 change of −3.7 to −5.2 log10 (PMID 20951424).\n\nThe last sentence of the claim is wrong: \"None of the reviewed papers reported an SVR endpoint for a polymerase-inhibitor regimen.\" The contribution itself cites PMID 18821613, which is Pockros et al., Hepatology 2008;48(4):1349-50, doi 10.1002/hep.22593. Its title is \"High relapse rate seen at week 72 for patients treated with R1626 combination therapy\". This is the post-treatment follow-up (week 72, meaning 24 weeks after the end of treatment) of the R1626 plus PEG-IFN/RBV regimen from PMID 18570306. So a durable post-treatment endpoint for a regimen containing a polymerase inhibitor was published in 2008. The already-settled result RC-4533 in this task reports those SVR24 values: 24%, 53% and 58% against 50% with standard care. Whether a durable endpoint was measured is exactly what T-24 asks, so this error is substantive, not a caveat. I could not open the letter's full text (Wiley blocked access, and it has no abstract), but its PubMed title alone contradicts the claim. The claim would be accurate if it said no interferon-free polymerase regimen had a measured SVR, and that the only durable data (the R1626 letter) showed high relapse.",

I re-fetched PubMed efetch abstracts for 18553458, 18570306, 20951424, 19072827, 19320234, 18821613 and 18285474, and opened natap.org/2008/HCV/100908_02.htm.\n\nWhat reproduces (all dated before 2011):\n- R1626 alone for 14 days: mean falls of 0.32, 1.2, 2.6 and 3.7 log10 (18553458).\n- R1626 plus PEG-IFN±RBV at week 4: 3.6, 4.5 and 5.2 log10 vs 2.4 on SOC. Grade 4 neutropenia was 48%, 78% and 39% vs 10% (18570306).\n- INFORM-1 (Lancet, epub 14 Oct 2010): median day-14 change of −3.7 to −5.2 log10. At the top dose it was −5.1 in naive patients and −4.9 in null responders (20951424).\n- HCV-796: abnormal liver tests in 8% (19072827).\n- BILB 1941: 5-day dosing and GI intolerance at 450 mg (19320234).\nThe evidence table itself is accurate.\n\nWhy the claim fails. Its last sentence reads: \"None of the reviewed papers reported an SVR endpoint for a polymerase-inhibitor regimen.\" That is false, and the error goes to the core of what T-24 asks: whether any durable cure endpoint was measured. The contribution cites PMID 18821613 as a source (Pockros et al., Hepatology 2008 Oct;48(4):1349-50, \"High relapse rate seen at week 72 for patients treated with R1626 combination therapy\"). Week 72 is 24 weeks after the end of treatment. The letter text on NATAP reports post-treatment relapse among end-of-treatment responders: 2/12 (17%) on SOC, 6/11 (55%) on DUAL1500 and 7/25 (28%) on TRIPLE1500. So a durable post-treatment endpoint for a regimen containing a polymerase inhibitor was published in Oct 2008. The contribution's own detail says so (\"post-treatment (week-72) outcomes were reported by Oct 2008\"), but the claim contradicts it.\n\nA narrower wording would be accurate: no SVR was measured for an interferon-free polymerase regimen, and none could be attributed to the polymerase inhibitor; the only durable data (R1626) showed high relapse. The claim as written does not say that.\n\nSecondary point: \"No treatment-emergent resistance was detected in these short studies\" is too broad for the non-nucleoside class. McCown et al., AAC 2008 (PMID 18285474, pre-2011), states that 14-day HCV-796 monotherapy \"rapidly selected for drug-resistant variants.\"\n\nI uphold the dispute.
