"claim_statement": "By 31 Dec 2010, peer-reviewed clinical reports of therapeutic HCV vaccines in chronically infected patients (IC41 peptide, recombinant E1, CIGB-230 DNA/core) showed that the vaccines were safe and induced HCV-specific T-cell or antibody responses, but none reported sustained viral clearance: HCV RNA stayed unchanged or fell only transiently (>1 log in 3 of 60 IC41 recipients), and IC41 added to peginterferon/ribavirin did not prevent relapse, so vaccination was not an evidenced route to cure.",

Most were hepatitis A/B vaccine responses in HCV patients, which I excluded as not HCV treatment. HCV therapeutic vaccine reports among them: 19866482, 19559112, 19017255, 18471515, 14578869, plus HLA-restricted personalized peptide phase 1 studies (17845828, 19604246), which I did not review in detail.\nS2 = (TG4040 OR GI-5005 OR Tarmogen OR ChronVac OR IC41 OR \"E1 vaccine\" OR Innovac) AND hepatitis C AND 1990:2010[dp] gave 11 hits. I found no peer-reviewed patient report for TG4040 or GI-5005 through 2010: only a GI-5005 drug review (PMID 19649991) and preclinical yeast immunogenicity work (PMID 17098335). IC41 healthy-volunteer studies (16581161, 20060945) are not in chronically infected patients, so I did not use them for efficacy.\nS3 = hepatitis C AND therapeutic vaccin*[tiab] AND (viral load OR HCV RNA) AND patients AND 2000:2010[dp] gave 16 hits and no further eligible primary clinical reports.\nACCESS LIMITATION: PubMed abstracts only; no full texts. Dates come from PubMed esummary.\n\nEVIDENCE TABLE\n1) IC41 in non-responders: Klade et al., Gastroenterology 2008;134:1385-95. DOI 10.1053/j.gastro.2008.02.058, PMID 18471515. Published online 4 Mar 2008; issue May 2008. Population: 60 HLA-A2+ chronic HCV patients who had not responded to or had relapsed after standard therapy. Design: double-blind randomized phase 2 with 5 arms: IC41 at 3 doses, peptides alone, or poly-L-arginine adjuvant alone. Regimen: 6 vaccinations. Results (abstract): T-cell proliferation in up to 67% on IC41 vs 17% on peptides alone; IFN-gamma ELISpot responses only in the IC41 arms, up to 42%; 3 of 60 patients had TRANSIENT HCV RNA declines of more than 1 log. No drug-related serious adverse events and no induced hepatitis. The authors state that HCV RNA \"changes ... occurred only in single patients\". SVR was not an endpoint.\n2) IC41 added to PegIFN/RBV: Wedemeyer et al., Vaccine 2009;27:5142-51. DOI 10.1016/j.vaccine.2009.06.027, PMID 19559112. Published online 24 Jun 2009. Population: 35 genotype 1 patients on standard therapy. Design: single arm, no randomized control mentioned in the abstract. Regimen: 6 IC41 doses between weeks 28 and 48 of PegIFN/RBV, then 6 months of follow-up. Results: IC41 \"did not prevent HCV-RNA relapse\"; HCV-specific T-cell responses rose in 73%, and were more frequent and stronger in patients with SVR than in those who relapsed. The abstract gives no SVR denominator. This is an association, not a causal effect.\n3) Recombinant E1: Nevens et al., Hepatology 2003;38:1289-96. DOI 10.1053/jhep.2003.50474, PMID 14578869. Published Nov 2003. Population: 35 genotype 1 patients (18 of the 24 biopsied had failed interferon-based treatment). Design: randomized, E1 (n=26) vs alum placebo (n=9), then open-label E1 for all. Regimen: two courses over about 65 weeks. Results: \"Plasma HCV-RNA levels remained unchanged\". Ishak score improved by 2 points or more in 9/24 (38%), was stable in 10 (41%) and worsened in 5 (21%); in this second phase there was no concurrent control. De novo E1-specific T-cell responses in all but 3 patients. Well tolerated.\n4) CIGB-230 DNA plus core protein: Alvarez-Lajonchere et al., J Viral Hepat 2009;16:156-67. DOI 10.1111/j.1365-2893.2008.01058.x, PMID 19017255. Published online 31 Oct 2008. Population: 15 genotype 1b non-responders to IFN/RBV. Design: phase 1, single arm. Regimen: 6 intramuscular doses over 20 weeks. Results: specific T-cell responses in 73% at week 24; new neutralizing antibody responses in 6 patients; HCV RNA persistently detected; histology improved or stable in more than 40%. Companion safety report: Castellanos et al., J Gene Med 2010;12:107-16 (DOI 10.1002/jgm.1407, PMID 19866482; issue Jan 2010, PubMed entrez 30 Oct 2009) found adverse events mild (83.6%) or moderate (16.4%) and no change in aminotransferases.\n\nCONCLUSION (<=250 words). Observed: every eligible report enrolled patients who were ALREADY chronically infected. These were therapeutic vaccines, not prophylactic ones, and I used no prophylactic data. The endpoints were safety, immunogenicity, HCV RNA kinetics and histology. None measured or reported SVR attributable to the vaccine. Viral effects were absent (E1, CIGB-230) or transient in a few patients (IC41: 3 of 60 with a >1-log decline), and IC41 given alongside PegIFN/RBV did not prevent relapse. Inference: by 2010, therapeutic vaccination had shown biological activity (inducible T-cell responses despite ongoing viraemia) but no evidence of clearing the virus. It lagged far behind the PegIFN/RBV baseline and behind protease inhibitor triple therapy, whose SVR results were published in phase 2 trials. At most it was a hypothetical adjunct.\n\nUNKNOWNS / UNTESTED by 2010-12-31: randomized SVR endpoints for any vaccine used with or instead of PegIFN/RBV; peer-reviewed patient data for vector vaccines (TG4040 MVA, GI-5005 yeast); whether vaccine-induced responses select immune-escape variants; the durability of histological changes; and results in patients who are not HLA-A2 (IC41 is HLA-restricted). No recommendation to any patient is made.",

Every figure matches; SVR indeed not an endpoint.\n2) PMID 19559112 (Vaccine 2009 Aug 13;27(37):5142-51, Epub 24 Jun 2009): 35 genotype 1 patients, 6 IC41 vaccinations weeks 28–48 of PegIFN/RBV, 6 months follow-up; \"IC41 vaccination did not prevent HCV-RNA relapse\"; T-cell increase in 73%; responses \"more frequent and stronger in patients with sustained virologic response than in patients who relapsed\". Matches, and the contribution is right that the abstract gives no SVR denominator and that this is association, not effect.\n3) PMID 14578869 (Hepatology 2003 Nov;38(5):1289-96): 35 genotype 1 patients randomized to E1 (n=26) or placebo (n=9) at weeks 0/4/8/12/24, then open-label E1 weeks 50–65; 24 biopsied, 18 interferon-based treatment failures; Ishak improvement >=2 points 9/24 (38%), stable 10 (41%), worse 5 (21%) — recomputed 37.5/41.7/20.8%, consistent; \"Plasma HCV-RNA levels remained unchanged\"; \"all but 3 patients developed a significant de novo E1-specific T-cell response\". Matches.\n4) PMID 19017255 (J Viral Hepat 2009 Mar;16(3):156-67, Epub 31 Oct 2008): CIGB-230 IM weeks 0/4/8/12/16/20 in 15 chronically infected IFN/RBV non-responders; neutralizing-antibody responses modified in 8 including 6 de novo; 73% with specific T-cell proliferative and IFN-gamma responses 24 weeks after primary immunization; \">40% ... improved or stabilized liver histology\" \"despite persistent detection of HCV RNA\". Matches. Matches (contribution writes \"mild\" for the abstract's \"slight\").\n\nDATES: first publication of all five is on or before 2010-12-31 (2003, 2008, 2008 Epub/2009 issue, 2009, 2010 Jan issue). No hindsight used. Absence of a peer-reviewed patient report for TG4040 or GI-5005 by the cutoff holds up in my re-run: 19649991 is a drug review, 17098335 is preclinical yeast work, and the only TG4040-era S2 hit outside the cutoff (21473061) is a 2010 Polish-language review, not a trial report.\n\nCLAIM CHECK: the claim statement is supported exactly — safety and induced T-cell/antibody responses in all three vaccine families, no reported sustained viral clearance, HCV RNA unchanged (E1, CIGB-230) or transiently >1 log in 3 of 60 IC41 recipients, and IC41 added to PegIFN/RBV not preventing relapse. The claim is scoped by its own parenthetical to IC41/E1/CIGB-230, so it does not overreach.\n\nTWO COMPLETENESS CAVEATS, neither contradicting the claim and neither affecting reproduction: (a) S1 also returns 18455842 (V-5 Immunitor oral therapeutic vaccine, 10 chronic HCV patients, 1 month, open-label), which the contribution neither reviews nor explicitly excludes; its abstract reports only ALT/AST falls and no anti-HCV seroreversion, with no HCV RNA outcome, so it adds no clearance evidence; (b) 19604246, which the contribution names but does not review, reports \"two HCV RNA responders with >1-log declines\" in personalized peptide vaccination — again transient declines in single patients, consistent with the conclusion. Abstracts only, same access limitation the contribution declares.\n\nSEARCH REPRODUCTION — all three bounded searches returned exactly the stated counts:\nS1 (\"hepatitis C, chronic\"[mh] OR \"chronic hepatitis C\"[tiab]) AND vaccin*[ti] AND clinical trial[pt] AND 1990:2010[dp] = 22 hits (stated 22). The five HCV therapeutic-vaccine reports it names (19866482, 19559112, 19017255, 18471515, 14578869) and the two HLA-restricted personalised-peptide phase 1 studies (17845828, 19604246) are all present in the returned ID list.\nS2 (TG4040 OR GI-5005 OR Tarmogen OR ChronVac OR IC41 OR \"E1 vaccine\" OR Innovac) AND hepatitis C AND 1990:2010[dp] = 11 hits (stated 11).\nS3 hepatitis C AND therapeutic vaccin*[tiab] AND (viral load OR HCV RNA) AND patients AND 2000:2010[dp] = 16 hits (stated 16).\n\nNEGATIVE FINDINGS CHECKED: an unrestricted PubMed search for TG4040 returns 6 records, earliest 21473061; esummary shows 21473061 is a 2010 Polish-language review of HCV inhibitors (Przegl Epidemiol), not a patient report, and every other TG4040 record is 2011 or later. GI-5005 returns 5 records: 19649991 is the Curr Opin Mol Ther drug review, 17098335 the preclinical whole-yeast immunogenicity paper, 20448862 is a \"Gateways to clinical trials\" bibliography digest, the rest are reviews. So \"no peer-reviewed patient report for TG4040 or GI-5005 through 2010\" holds. The exclusion of IC41 healthy-volunteer studies 16581161 and 20060945 is correct — esummary titles confirm both enrolled healthy subjects, not chronically infected patients.\n\nFACTS RE-OPENED AND MATCHED, all publication dates on or before 2010-12-31:\n1) Klade, Gastroenterology 2008 May;134(5):1385-95, Epub 2008 Mar 4, PMID 18471515. Abstract confirms 60 HLA-A2-positive non-responder/relapser patients, double-blind phase II, 5 groups, 6 vaccinations; proliferation \"up to 67%\" on IC41 vs 17% on peptides alone; IFN-gamma ELISpot \"exclusively in the IC41 groups\" up to 42%; \"3 RNA responders with transient >1-log declines\"; \"no drug-related serious adverse events or induction of hepatitis\"; \"changes in HCV RNA occurred only in single patients\". SVR is not an endpoint. Every figure matches.\n2) Wedemeyer, Vaccine 2009;27(37):5142-51, Epub 2009 Jun 24, PMID 19559112. Confirms 35 genotype 1, 6 IC41 doses weeks 28-48 of PegIFN/RBV, 6 months follow-up, \"IC41 vaccination did not prevent HCV-RNA relapse\", T-cell increase in 73%, responses \"more frequent and stronger in patients with sustained virologic response than in patients who relapsed\". No SVR denominator in the abstract and no randomised control described — the contribution's reading of this as association, not causal effect, is correct.\n3) Nevens, Hepatology 2003 Nov;38(5):1289-96, PMID 14578869. Confirms 35 genotype 1 randomised to E1 n=26 vs alum placebo n=9 (weeks 0,4,8,12,24) then 34 open-label weeks 50-65, so \"two courses over about 65 weeks\" is right; 24 biopsied, of whom 18 were interferon-based treatment failures; \"Plasma HCV-RNA levels remained unchanged\". Percentages recomputed from denominators: 9/24 = 37.5% (38%), 10/24 = 41.7% (41%), 5/24 = 20.8% (21%) — all as reported. \"All but 3 patients developed a significant de novo E1-specific T-cell response\" matches.\n4) Alvarez-Lajonchere, J Viral Hepat 2009 Mar;16(3):156-67, Epub 2008 Oct 31, PMID 19017255. Confirms 15 genotype 1b IFN/RBV non-responders, phase I, 6 IM doses weeks 0-20; 73% T-cell proliferative and IFN-gamma response at 24 weeks; neutralising-antibody responses modified in 8 including 6 de novo responders; \"despite persistent detection of HCV RNA, more than 40%\" improved or stabilised histology.\n5) Castellanos, J Gene Med 2010 Jan;12(1):107-16, PMID 19866482. Confirms adverse events \"only slight (83.6%) or moderate (16.4%)\" and no significant change in serum aminotransferases.\n\nNo hindsight: nothing in the answer uses post-2010 material, and no clinical recommendation is made. The conclusion does not turn short-term RNA decline into cure — it says the opposite.\n\nWHY PARTIAL, not confirmed. One descriptor in the claim sentence is not supported exactly by the source. The claim reads \">1 log in 3 of 60 IC41 recipients\". Klade's 60 patients were randomised across five arms, only three of which received IC41; the abstract places the 3 RNA responders \"within all 60 patients\", not within IC41 recipients. The contribution's own detail text states this correctly (\"3 of 60 patients\", \"within all 60 patients\"), so this is a slip in the settling sentence rather than a defect in the work. The error is conservative in direction (3/60 = 5% understates the rate among the roughly 36 IC41 recipients) and changes no part of the conclusion, which is why I am not disputing: the work reproduced in full. But because a confirmed claim becomes the project's belief, the denominator should read \"3 of 60 randomised patients\" or \"3 of 60 patients in the IC41 trial\" before it settles. An amendment to that effect would, on this evidence, deserve a confirm.\n\nTwo smaller notes, neither affecting the verdict. The S2 narrative accounts for only some of the 11 hits; among the unmentioned ones, 17038823 (Hum Vaccin 2005, intradermal HCV E1 candidate in healthy volunteers and patients with resolved or ongoing chronic HCV, indexed as Clinical Trial) and 17686555 (Vaccine 2007, peptide-vaccine-induced HCV-specific CD8+ T cells in healthy individuals and chronic hepatitis C patients, indexed as Controlled Clinical Trial) do include chronically infected patients and are arguably eligible. Both are immunogenicity/tolerability reports without viral-clearance endpoints, so including them would reinforce rather than weaken the conclusion, and the task caps the review at four primary reports, which this answer meets. I could not check the artefact vax_abstracts.txt, which was listed without bytes and is not retrievable from here; I regenerated the five abstracts myself instead.",
