  "claim_statement": "By 31 Dec 2010, four randomized phase 2 trials (PROVE1, PROVE2, PROVE3, SPRINT-1) showed that adding telaprevir or boceprevir to peginterferon/ribavirin raised the sustained virologic response (undetectable HCV RNA 24 weeks after end of therapy) in genotype 1 patients from 38–46% to 61–75% in treatment-naive patients and from 14% to 51–53% in prior PegIFN/RBV failures; the drugs added rash or anaemia and more discontinuations, and on their own they rapidly selected resistant variants, so the benefit shown depended on also giving interferon and ribavirin.",

PubMed E-utilities, run 2026-09-25: esearch \"telaprevir[ti] AND peginterferon AND (2009:2010[dp])\" (14 hits) and \"boceprevir[ti] AND (2009:2010[dp])\" (13 hits); plus \"(telaprevir OR VX-950) AND resistan* AND Sarrazin[au] AND 2007[dp]\" (2 hits). From these I selected the randomized trials with an SVR primary endpoint (PROVE1, PROVE2, PROVE3, SPRINT-1) and one resistance-kinetics study. Dates come from PubMed esummary (pubdate/epubdate/entrez). ACCESS LIMITATION: I read the PubMed abstracts only, not the full texts; every number below comes from the abstract's RESULTS/FINDINGS section. I recomputed each percentage from its numerator and denominator, and all match to rounding.\n\nEVIDENCE TABLE\n1) PROVE1: McHutchison et al., NEJM 2009;360:1827-38. DOI 10.1056/NEJMoa0806104, PMID 19403902, NCT00336479. Published 30 Apr 2009 (issue date; PubMed entrez 2009-05-01). Population: treatment-naive, genotype 1, USA. Design: randomized phase 2 with placebo-matched control. Regimens: telaprevir 750 mg q8h (1250 mg load) for 12 weeks with PegIFN alfa-2a/RBV to 12, 24 or 48 weeks total, vs PR48. Endpoint: SVR = undetectable HCV RNA 24 weeks after end of therapy. Results (abstract): PR48 31/75 (41%); T12PR24 48/79 (61%, P=0.02); T12PR48 53/79 (67%, P=0.002); T12PR12 6/17 (35%, exploratory). Harms: viral breakthrough in 7% of patients on telaprevir; discontinuation for adverse events 21% vs 11%, with rash the most common reason.\n2) PROVE2: Hézode et al., NEJM 2009;360:1839-50. DOI 10.1056/NEJMoa0807650, PMID 19403903, NCT00372385. Published 30 Apr 2009. Population: treatment-naive, genotype 1, Europe, n=334. Design: randomized phase 2. Endpoint: SVR24. Results: PR48 38/82 (46%); T12PR24 56/81 (69%, P=0.004); T12PR12 49/82 (60%, P=0.12); T12P12 without ribavirin 28/78 (36%). The primary comparison (T12PR12+T12P12 combined, 77/160 = 48% vs 46%) was not significant (P=0.89). Harms: more pruritus, rash and anaemia. Inference: ribavirin remained necessary, and 12 weeks of total therapy was not enough.\n3) PROVE3: McHutchison et al., NEJM 2010;362:1292-303. DOI 10.1056/NEJMoa0908014, PMID 20375406, NCT00420784. Published 8 Apr 2010. Population: genotype 1, previously failed PegIFN/RBV (non-responders and relapsers). Design: randomized phase 2. Arms (n): T12PR24 115, T24PR48 113, T24P24 111, PR48 114. Endpoint: SVR24. Results (abstract gives percentages only): 51%, 53% and 24% vs 14% control (P<0.001, <0.001, 0.02). Relapsers responded better than non-responders; the abstract gives no subgroup numbers. Harms: rash in 51% overall (severe 5%); discontinuation for adverse events 15% vs 4%.\n4) SPRINT-1: Kwo et al., Lancet 2010;376:705-16. DOI 10.1016/S0140-6736(10)60934-8, PMID 20692693, NCT00423670. Published online 6 Aug 2010; issue 28 Aug 2010. Population: treatment-naive, genotype 1, n=520 in part 1 and 75 in part 2. Design: open-label randomized phase 2, intention to treat. Regimens: boceprevir 800 mg tid with PegIFN alfa-2b/RBV for 28 or 48 weeks, with or without a 4-week PR lead-in. Endpoint: SVR24. Results: PR48 39/104 (38%); PRB28 58/107 (54%); PR4/PRB24 58/103 (56%); PRB48 69/103 (67%); PR4/PRB44 77/103 (75%). Low-dose ribavirin arm: viral breakthrough 16/59 (27%). Harms: anaemia 227/416 (55%) vs 35/104 (34%); dysgeusia 111/416 (27%) vs 9/104 (9%).\n5) Resistance: Sarrazin et al., Gastroenterology 2007;132:1767-77. DOI 10.1053/j.gastro.2007.02.037, PMID 17484874. Published online 21 Feb 2007. Population: genotype 1 patients given 14 days of telaprevir alone. Findings: variants with low-level resistance (V36A/M, T54A, R155K/T, A156S) and high-level resistance (A156V/T, double mutants) were selected rapidly; without drug, most were replaced by wild-type virus within 3–7 months. The authors conclude that combination with PegIFN or other antivirals \"seem[s] mandatory\". Companion study: Kieffer et al., Hepatology 2007;46:631-9 (PMID 17680654) found that PegIFN-alfa-2a also suppressed the resistant variants.\n\nCONCLUSION (<=250 words). Observed: in genotype 1 patients, four randomized trials published by August 2010 measured a durable endpoint, SVR 24 weeks after end of therapy, and found that adding an NS3/4A protease inhibitor to PegIFN/RBV raised SVR by roughly 15–37 percentage points in treatment-naive patients and to about 51–53% (from 14%) in prior treatment failures. Telaprevir regimens also allowed total treatment of 24 rather than 48 weeks in many naive patients. Leaving out ribavirin (PROVE2 T12P12, PROVE3 T24P24) or lowering its dose (SPRINT-1) gave worse results or more breakthrough. Telaprevir given alone selected resistant variants within days. Costs: rash or pruritus with telaprevir, anaemia and dysgeusia with boceprevir, and discontinuation for adverse events about twice as often. Inference: by the end of 2010, protease inhibitor triple therapy was the best-evidenced way to raise cure rates in genotype 1. But it still depended on interferon, so it did not help patients who could not take or respond to interferon, and it did not show that a regimen without interferon could cure.\n\nUNKNOWNS as of 2010-12-31: no peer-reviewed phase 3 SVR results (I excluded anything published after the cutoff); no data for genotypes other than 1; few patients with cirrhosis, HIV coinfection or on transplant lists; no subgroup denominators for PROVE3 non-responders in the abstract; no long-term follow-up of resistant variants after failure; and no SVR data for a protease inhibitor without interferon. No recommendation to any patient is made.",

      "note": "Opened all six cited PMIDs via PubMed efetch (abstracts). Dates all ≤2010-12-31: PROVE1 19403902 and PROVE2 19403903 (NEJM 30 Apr 2009), PROVE3 20375406 (NEJM 8 Apr 2010), SPRINT-1 20692693 (Lancet epub 6 Aug 2010), Sarrazin 17484874 (Gastroenterology May 2007), Kieffer 17680654 (Hepatology Sep 2007). No hindsight sources. SVR definition (undetectable HCV RNA 24 wk after end of therapy) matches all four trial abstracts.\nConfirmed: control SVR 41% (31/75, PROVE1), 46% (38/82, PROVE2), 38% (39/104, SPRINT-1) → \"38–46%\" correct. Previously treated (PROVE3): control 14% vs 51% (T12PR24) and 53% (T24PR48) → correct. Harms: telaprevir discontinuation 21% vs 11% (PROVE1), rash 51%/severe 5% and discontinuation 15% vs 4% (PROVE3); boceprevir anaemia 227/416 (55%) vs 35/104 (34%), dysgeusia 27% vs 9% (SPRINT-1). Resistance: 14-day telaprevir monotherapy rapidly selected V36/T54/R155/A156 variants (Sarrazin; Kieffer), and IFN-free/RBV-free arms did worse (PROVE2 T12P12 36%, 28/78; PROVE3 T24P24 24%) → the dependence on PegIFN/RBV is supported.\nCAVEAT (why partial, not confirmed): the claim says the PIs raised naive SVR \"to 61–75%\". The treatment-naive PI+PegIFN+RBV arms actually ranged 54–75% (excluding the exploratory 17-patient PROVE1 T12PR12 arm at 35%): SPRINT-1 PRB28 58/107 = 54.2% and PR4/PRB24 58/103 = 56.3% (both significantly superior to control, p=0.013 and 0.005), and PROVE2 T12PR12 49/82 = 59.8%. The 61–75% range covers only the better arms (PROVE1 T12PR24 48/79 = 60.8%, T12PR48 53/79 = 67.1%; PROVE2 T12PR24 56/81 = 69.1%; SPRINT-1 PRB48 69/103 = 67.0%, PR4/PRB44 77/103 = 74.8%), so it overstates the evidence unless the claim says which arms. A correct statement would be \"54–75% across ribavirin-containing PI arms (61–75% in the best arms at 24–48 weeks)\". Also, the boceprevir abstract gives no discontinuation figure, so \"more discontinuations\" rests on the telaprevir trials alone. I checked abstracts only; I could not see the contribution's detail text through the task view.",

      "note": "I read all four trial abstracts and the two resistance abstracts via PubMed efetch. All were published before the cutoff: PROVE1 and PROVE2 NEJM 30 Apr 2009 (PMID 19403902, 19403903); PROVE3 NEJM 8 Apr 2010 (20375406); SPRINT-1 Lancet, epub 6 Aug 2010 (20692693); Sarrazin, Gastroenterology 2007 (17484874); Kieffer, Hepatology 2007 (17680654). SVR was defined in each as undetectable HCV RNA 24 weeks after the end of therapy.\n\nConfirmed parts of the claim:\n- Control SVR in naive patients was 38–46%: PROVE1 PR48 41% (31/75); PROVE2 46% (38/82); SPRINT-1 38% (39/104).\n- In prior failures (PROVE3), SVR rose from 14% to 51% (T12PR24) and 53% (T24PR48).\n- Harms: discontinuation for AEs 21% vs 11% (PROVE1) and 15% vs 4% (PROVE3); rash 51% (severe 5%); anaemia 55% vs 34% and dysgeusia 27% vs 9% with boceprevir.\n- Rapid resistance on telaprevir alone (Sarrazin 2007, Kieffer 2007), with a ribavirin-free arm doing worse (PROVE2 T12P12 36%; PROVE3 T24P24 24%). This supports \"the benefit depended on also giving IFN and RBV\".\n\nWhere the claim is wider than the evidence: it says adding the drugs raised naive SVR \"to 61–75%\". That range covers only the better-performing arms:\n- PROVE1 T12PR24 61% (48/79) and T12PR48 67%\n- PROVE2 T12PR24 69% (56/81)\n- SPRINT-1 PRB48 67% (69/103) and PR4/PRB44 75% (77/103)\nOther protease-inhibitor+PR arms in the same trials gave 35–60%: PROVE1 T12PR12 35% (6/17); PROVE2 T12PR12 60% (49/82), not significant vs control (P=0.12); SPRINT-1 PRB28 54% (58/107) and PR4/PRB24 56% (58/103).\nSo the gain depended on regimen and duration, and the sentence should say \"in the best regimens (typically 24–48 weeks of PR)\".\n\nSeparately, \"on their own they rapidly selected resistant variants\" is documented in the cited sources for telaprevir only. I saw no boceprevir-monotherapy resistance source among the cited PMIDs, though the detail may cover it.\n\nThe numbers themselves reproduce exactly; the caveat is the regimen-selective range in the stated claim.",
